Provide the narrative in paragraph form in the text field provided. Please be thorough but strive for brevity. 1. Briefly describe the organization’s history, mission and goals. The Maricopa County Department of Public Health (MCDPH) serves the third largest local public health jurisdiction in the U.S., with more than 4.1 million residents according to the 2015 census. Maricopa County encompasses over 9,200 square miles, roughly the size of the state of Massachusetts, composed of a mix of urban, suburban, and rural areas including the whole or parts of five sovereign American Indian Reservations. Home to the major metropolitan cities of Phoenix, Scottsdale, Mesa, Glendale, and Tempe, Maricopa County serves as the state’s major geopolitical and economic center. Our mission is to protect and promote the health and well-being of all of our residents and visitors. This means providing protection from the health threats such as food-borne illnesses, natural and man-made disasters, toxic exposures, and preventable injury. Public Health also works to prevent chronic diseases, such as heart disease, cancer, and diabetes by addressing their risk factors: poor nutrition, inadequate physical activity, and tobacco use. We are increasingly moving towards policy, systems and environmental approaches to improving the health of our community. As a partner of the Arizona Departmant of Health Services we have a long history of implementing innovative and effective health promotions within Maricopa County. One of our goals as a large health department is to implement self-management programs to populations who have a need for Diabetes and/or Chronic Disease education with the combined goal of increasing the self-efficacy of completers, reducing costs associated with healthcare and reducing preventable hospital readmissions within Maricopa County. 2. If the applying organization is non-municipal or a non-profit 501c, please describe your relationship with Arizona municipalities including past and present support, whether it be through partnerships, Gila River Indian Community Grant Application Grant Cycle 2017 collaborations, in-kind contributions, grants, tax-based funding or other. Describe the supporting municipality’s role in this project. If the applying organization is a municipality governmental sub- division please enter the text “Not applicable.” Not applicable. 3. Describe the proposed project and planned goals and activities. From cancer and stroke to diabetes and Alzheimer’s, seven out of the 10 leading causes of death in Arizona are attributed to a chronic disease. Regardless of race or ethnic group, arthritis is the number one cause of disability, with one in four adults receiving a diagnosis from a provider. The Stanford Model of evidence-based self-management programs specific to chronic disease and diabetes education are a key way to help Arizonans lead longer, healthier lives and has a proven return on investment that decreases the amount of health related expenditures related to a patient or paticpant’s health care. However, many do not know that this opportunity exists within our community and are not referred to workshops by a healthcare providers. The Healthy Living Initiative of Maricopa County will help bridge that gap in access to self- management workshops by empowering community partners to deliver this evidence-based program within Maricopa County. Planned goals: Goal #1: To create partnerships that result in the facilitation of Chronic Disease Self- Management (CDSMP) and/or Diabetes Self-Management (DSMP) workshops per grant year (self-management workshops can be provided in both English or Spanish, depending on the audience’s needs). Goal #2: To train 30 lay-leaders of CDSMP and/or DSMP workshops per grant year. Goal #3: To deliver workshops of the Stanford model of self-management curriculum by creating partnerships that result in the facilitation of 30 CDSMP and/or DSMP workshops per grant year. Goal #4: Provide technical assistance for workshop implementation that includes activities related to workshop logistics and support to include: Workshop Books, Workshop Coordination Stipends, Workshop Lay Leader Training and workshop Nutrition Support through Farmer’ Market produce support for attendees of self-management workshops Activities: Gila River Indian Community Grant Application Grant Cycle 2017 (1). Develop partnerships to assist in reaching community members who can benfit from self- management workshops within Maricopa County. (2). Create infrastructure to support self-management education to community members. (3). Create a network of self-management partners and lay-leaders to support workshop delivery in designated communities within Maricopa County. (4). Train lay-leaders to conduct CDSMP and DSMP workshops. (5). Provide support and technical assistance to partners who contribute to the implementation of self-management workshops within Maricopa County. (6.) Employing the use of nutritional support activities by providing workshop participants with an opportunity to receive fresh produce as a benefit of completing the workshop. The one-time issue of nutrition related items will fortify the nutritional component of the self-management workshops employed throughout Maricopa County. 4. Describe how the proposed project satisfies one or more of the priority funding areas identified by the Gila River Indian Community. Healthcare – The proposed project features self-management of chronic disease and diabetes education workshops. These evidence-based workshops promote an increase in self-efficacy in those who are at risk or are suffering from chronic disease and/or diabetes. The Stanford Model of self- management workshops are designed to assist in improving the overall health of participants. 5. Identify the needs/problems to be addressed, target population and number of people to be served by the project. Problems to be addressed: According to data obtained through the Behavioral Risk Factor Surveillance System, a self- reporting survey, (1) high blood pressure and cholesterol are still major contributors to the national epidemic of cardiovascular disease. High blood pressure affects approximately 1 in 3 adults in the United States, and more than half of Americans with high blood pressure do not have it under control. High sodium intake is a known risk factor for high blood pressure and heart disease, yet about 90 percent of American adults exceed their recommendation for sodium intake. (2) Diabetes was ranked the first most important health problem by community members (African American, American Indian, Asian American, and Hispanic) in the Maricopa County Community Health Survey, 2012. By developing partnerships with health networks, the patient population in the healthcare footprint and surrounding communities will be educated on the concept of self-management of chronic disease and/or Diabetes. This targeted education will directly decrease the cost burden of those health network partners as target populations increase their health literacy and self- efficacy in regards to their own health. This will create better health related outcomes for participants, while a return on investment is realized through cost savings from the changed behaviors of participants. The number of people to be served by this project will be in excess of Gila River Indian Community Grant Application Grant Cycle 2017 1000 (1080 participants projected). 6. Define the project as a new or continuing program. This is a new project that is designed to implement self-management through partnerships with community centers of influence within Maricopa County. 7. Provide a brief timeline including start and finish dates. Indicate if the timeline is flexible. 10/01/2017 - Work will begin with the creation of partnerships and lay-leader training activities to support workshop volume that is projected. - Partnerships will be fostered - Creation of Lay leader cadre will take place Years # 1-3 - Partnerships, training and workshop implementation activities to support program goals will be implemented throughout all three years of this grant proposal. The project will conclude on 9/30/2020 with completion of project objectives. - The timeline is flexible. 8. Identify other organizations, partners or funders participating in the project and their roles. (1). The Arizona Living Well Institute is the state endorsed vendor of self-management workshop implementation in the state. They will provide workshop fidelity and training in support of self-management workshop implementation and training. 9. Would you be able to implement the proposed plan if your organization received partial funding for this project? No 10. Describe your plan for project sustainability beyond the grant period. If this is a program/project previously funded by the Gila River Indian Community describe efforts made towards the previously described sustainability plan. The Arizona Living Well Institute has been historically supported by the Arizona Department of Health Services; funding is expected to continue. By training staff and/or volunteers in communities within Maricopa County to deliver self-management workshops, we are building the capacity of organizations and communities to deliver self-management education to their own residents/population. Lay-leaders who are trained will continue to be engaged in the future to deliver self-management education through a community referral network. The benefit of cost savings, the provision of workshop books will be coupled with technical support of MCDPH in order to sustain the delivery of self-management Gila River Indian Community Grant Application Grant Cycle 2017 workshops within Maricopa County, this will create the environment necessary to delivery further workshops well into the future, in support of community partners. 11. Describe your plan to document progress and results. Progress: (1). Increases in self-efficacy will be measured by pre and post survey for every participant. This is a key evaluation tool that measure the self-confidence of workshop participants. (2). Workshop coordination and support activities will be the responsibility of the Community Health Development Coordinator and will be electronically managed through existing infrastructure existing within the Community Health Development Team of the MCDPH. Results: The following results are anticipated: (1). 360 workshop completers per year (x3) = 1180 total completers during grant period (2). 30 Lay-leaders trained per year (x3) = 90 total lay-leaders trained during grant period (3). 30 Workshops implemented per year (x3) = 90 Workshops completed during grant period (4). Significant cost savings will be anticipated based on the completion of workshops by participants (5). Partnership satisfaction will be assessed throughout the process of the project, 12. Indicate any application to and/or awards made by a tribe other than the Gila River Indian Community for state shared revenues for this and any other project for the past five (5) years.
