Narrative
Please structure your proposal to provide the following information in the order indicated. Provide the narrative in
paragraph form in the text field provided. Please be thorough but strive for brevity.
1. Briefly describe the organization’s history, mission and goals.
In 2013, The Van Norman brothers, Kevin and Jeff, founded Opportunity4Kids with the mission to
change the lives of Arizona Youth and promote equal opportunity for all children during out-of-school
time; participation would be supported through financial assistance, equipment and uniforms. During
his 15 years of coaching, Kevin frequently discovered children that lacked the funds to purchase sporting
equipment, such as helmets, gloves, mouth pieces, etc. needed to participate. Additionally, there were
children that simply did not have the financial resources to pay for registration fees required by most
out-of-school time activities. Thus, the creation of Opportunity4Kids as a nonprofit, with the main goal
to provide support to children who would otherwise not participate due to lack of resources available. It
is through fostering sporting and extracurricular activity while promoting healthy and active lifestyles
that we will further the mental and social development of children through this much needed Out-of-
School Time Project.
2. If the applying organization is non-municipal or a non-profit 501c, please describe your relationship with Arizona
municipalities including past and present support, whether it be through partnerships, collaborations, in-kind
contributions, grants, tax-based funding or other. Describe the supporting municipality’s role in this project. If
the applying organization is a municipality governmental sub-division please enter the text “Not applicable.”
Opportunity4Kids has been a 501(c)(3) nonprofit since 2013. We provide financial support to
underprivileged youth and their families living throughout Maricopa County, the primary locale which
we serve. Since 2013, Opportunity4Kids has been included in the Maricopa County Charitable
Contribution Campaign and Scottsdale Charitable Contribution Campaign.
3. Describe the proposed project and planned goals and activities.
The proposed project will impact the lives of at least 110 children in Maricopa County with the following goals
through two specific activities:
a.) To provide an opportunity for children to engage in safe, structured physical activity during out-of-school
time through Skate After School. We would host this Youth Recreation Program where Skate After School
instructors supply hands on skateboarding training, techniques and activities to underprivileged children
while also furnishing the necessary safety wear, helmets and equipment needed to conduct the instruction.
The proposed project provided an entire year of Skate After School Recreational Programming, broken down
into 3 semesters to mirror the school calendar year- Spring, Summer and Fall Semester. Each semester
program will serve a minimum of 30 children per week.
b.) To provide resources for children to engage in out-of-school time activities/extracurriculars of their interest.
By promoting healthy, active lifestyles, participating children will enhance their mental and social

Gila River Indian Community Grant Application
Grant Cycle 2017
development thus positively impacting their lives. We would facilitate these opportunities by providing
Youth Recreation Scholarships to pay for registration fees required to participate; or by providing equipment
and uniforms as needed by children participating in out-of-school time activities. Opportunity4Kids will
encourage the healthy behaviors of youth residing throughout Maricopa County by providing financial
support in the area of Healthcare; more specifically by making recreational activities that promote physical
fitness and wellness accessible during out-of-school time.
4. Describe how the proposed project satisfies one or more of the priority funding areas identified by the Gila River
Indian Community.
Opportunity4Kids will encourage the healthy behavior of youth residing throughout Maricopa County by
providing financial support in the area of Healthcare; more specifically by making recreational activities
that promote physical fitness and wellness accessible during out-of-school time.
5. Identify the needs/problems to be addressed, target population and number of people to be served by the
project.
Per the US Census Bureau’s 2015 American Community Survey, in Maricopa County there were
1,014,531 children under the age of 18 years old in households and 753,436 enrolled in school. The
percentage of children living in households at or below the poverty line is 24.4% and it was determined
that 28.7% of children are living in households with supplemental security income, cash public
assistance income or receiving food stamp benefits. Opportunity4Kids is a much-needed resource with
its main goal to provide assistance to children ages 5-18 years old, who are students enrolled in the
Maricopa County school district. We will target a minimum of 110 unique youth that would otherwise
not be involved in out-of-school time activities due to their financial circumstances.
6. Define the project as a new or continuing program.
Grant funding will supplement the organization's annual goals of providing financial resources that will
change the lives of children enrolled in schools within Maricopa county. Since 2013, Opportunity4Kids
has provided out-of-school time activities and resources for children, with over 1,000 children positively
impacted in 2016. Awarded this additional funding, Opportunity4Kids will dramatically increase youth
enrollment in out-of-school time recreational activities. There is a desperate need for this project in
Maricopa County.
7. Provide a brief timeline including start and finish dates. Indicate if the timeline is flexible.
Opportunity4Kids’ Director of Operations oversees general organization operations and is the only paid
personnel operating with the oversight of a volunteer Board of Directors.

Grant Cycle 2017
Beginning November 2017, an agreement contract with Skate After School and programming calendar
will be developed by the Director of Operations. Skate After School Programming will begin in January of
2018 with the start of Spring semester, concluding around the end of May/beginning of June. The
second semester of Skate After School programming will start in beginning of June for Summer
semester, concluding at the end of July/beginning of August. The third semester of Skate After School
programming will start at the beginning of August for Fall semester, concluding at the end of November.
Payment for programming cost will be submitted to Skate After School before the start date of each
semester.
It is through the word-of-mouth that Opportunity4Kids will solicit underprivileged youth to provide out-
of-school time recreational programming Scholarships. Parent/Guardian/Case Workers of all qualified
children will be required to complete an online application vetted by the Director of Operations and
then reviewed and processed by a designated Scholarship Approval Committee. The Youth Recreation
Scholarship Program will be implemented on a case by case timeline throughout the grant period of
2017-2018. The application process is directly utilized to identify the true need of applicants by
requesting financial and health information to determine that those awarded are underprivileged,
chronically ill or disabled.
8. Identify other organizations, partners or funders participating in the project and their roles.
Opportunity4Kids has established a supportive network of partners throughout Maricopa County by
building relationships with recreational organizations that provide influential out-of-school time
activities such as: the Boys and Girls Clubs of Metropolitan Phoenix, Skate After School, The Valley of the
Sun YMCA, City of Phoenix Recreational Activities, ATA Martial Arts, Sereno Soccer Club, Phoenix Center
for the Arts, Scottsdale Soccer, San Marcos Music Academy, ProCamps, Ballet Arizona, The Hubbard
Family Swim School, The Little Gym and many more.
These organizations support Opportunity4Kids’ goals of providing financial resources by discounting or
reducing registration fees required to participate facilitating the Scholarship of more children.
9. Would you be able to implement the proposed plan if your organization received partial funding for this project?
The proposed Out-of-School Time Project is a supplemental funding request. This is a much-needed
project and we hope to provide substantial financial assistance for the underprivileged children of
Maricopa County. We will increase youth enrollment in out-of-school time activities by at least 110 to

Gila River Indian Community Grant Application

specifically those who are at most in need of financial help. Opportunity4Kids will track household
location of those who participate to pinpoint specific areas of higher need for future targeting. The data
gathered will be used to further impact the lives of children who would not otherwise participate due to
the lack of financial resources available to them and their families who we serve.
10. Describe your plan for project sustainability beyond the grant period. If this is a program/project previously
funded by the Gila River Indian Community describe efforts made towards the previously described
sustainability plan.
This is our first time applying for a grant opportunity through the Gila River Indian Community. This is a
unique and much needed project; due to increasing costs of out-of-school time activities and childcare,
combined with the expanding population of children living below the poverty line, the number of
children needing access to out-of-school time activities has grown. Providing these opportunities to
children at risk and in need will very positively impact their lives and the community. It has thus been
identified that there is a real need for this type of organizational resource serving underprivileged youth
in Maricopa County. We are seeking this as a supplemental funding grant. Our end goal is to someday
provide this type of support to all youth at need. To make this goal attainable, Opportunity4Kids
conducts ongoing fundraising efforts, culminating with our annual VIP Dinner.
11. Describe your plan to document progress and results.
To identify the success of Scholarships provided, Opportunity4Kids requires a narrative from the
referring Parent/Guardian/Case Worker on the behalf of the child’s mental/physical/emotional state. In
addition, the Parent/Guardian/Case Worker will complete a post survey to determine individual
progress and development. Through the Scholarship application process, Opportunity4Kids’ Director of
Operations will utilize a tracking system to record information on children and their families applying for
Scholarship. Information gathered will include- school enrollment, age of participating children,
participation in government financial assistance programs, family financial situation, disabilities and
individual stories. The expectation is that 100% of children provided Scholarships will fall within 1.5% of
the federal poverty guidelines and at least 85% also receive additional government assistance.
Furthermore, 90% will show increase in self-esteem and social skills due to participation in
programming.

Opportunity4Kids works directly with Skate After School in determining the growth of participants
involved in this specific out-of-school time recreational activity. Youth will participate in pre-and post-
evaluation studies to assess the progression of skill and understanding of Skate After School core values.
After each session, the expected outcome is that 80% of students will show a basic understanding of the
parts of a skateboard and riding techniques. Community groups, in collaboration with the program, will
also be used to gather anecdotal evidence on the benefits of programming.
12. Indicate any application to and/or awards made by a tribe other than the Gila River Indian Community for state
shared revenues for this and any other project for the past five (5) years. If this information is included in a
separate attachment, please indicate that here.
For the period of 2015-2016, Opportunity4Kids was a recipient of $35,000 from a 12% Tribal Gaming
Grant through the Salt River Pima Maricopa Indian Community. Maricopa County Supervisor Steve
Chucri supported our application for that award.

Project Budget Detail
Please provide a narrative description for each of the project budget items listed on the previous page. Include the
dollar figure and how it was derived.
1. This line item includes the cost to provide Youth Recreation Scholarships, for the out-of-school time activity of their
choice, to at minimum 20 youth participants at an average Scholarship cost of $207.00 per child.
Therefore, 20 x $207.00/youth = $4,140.00
2. This line item includes the fee to provide the Out-of-School Time Youth Recreation Program, Skate After School.
Programming would take place in 3 designated semesters during FY2017-2018 (running Spring 2018, Summer 2018 and
Fall 2018).
Therefore, 3 x $1,500.00/program semester = $4,500.00
3. This line item includes costs for uniforms provided to the 90 participating youth in the Skate After School program
over the 3 semesters. Each T-Shirt order costs $280.00.
Therefore, 3 shirt orders x $280.00 = $840.00
4. This line item refers to the amount of Opportunity4Kids’ Director of Operations salary expense that can be allocated
to this specific project. This expense is completely funded by other sources. We are not requesting funds from this grant
to pay for this expense.
5. This line item refers to the in-kind volunteer hours dedicated to Skate After School programing. On a weekly basis, we
expect at least 7 hours of volunteer time valued at $23/hour for the entirety of the FY2017-2018; totaling 364 hours and
$8,372.00.
