Provide the narrative in paragraph form in the text field provided. Please be thorough but strive for brevity. 1. Briefly describe the organization’s history, mission and goals. We began in 2008 as a ministry of St. Timothy Catholic Church serving food to migrant workers in the fields and evolved to an independent 501(c)(3) non-profit organization. In 1994 we established our own facility located at Broadway and Arizona Avenue. Since then we have been serving evening meals and distributing food boxes daily. Our mission at Paz de Cristo is to feed, clothe and empower those in need in the Phoenix East Valley. Much of our resources are devoted to providing healthy and nutritious meals to the poor and under served in the East Valley. Those individuals or families experiencing food insecurity can come to a well-known, safe and reputable community to receive a hot meal nightly or to receive a food box. In addition to our food programs we provide showers and haircuts for the homeless, bus tickets for medical appointments or travel to a job , gift cards for medical prescriptions, vouchers for Arizona ID's and clothing for men, women and children. 2. If the applying organization is non-municipal or a non-profit 501c, please describe your relationship with Arizona municipalities including past and present support, whether it be through partnerships, collaborations, in-kind contributions, grants, tax-based funding or other. Describe the supporting municipality’s role in this project. If the applying organization is a municipality governmental sub-division please enter the text “Not applicable.” n/a 3. Describe the proposed project and planned goals and activities. Our proposed project is for funding to supplement our ongoing hunger program with an emphasis on healthy meals. We know from studies that obesity, health issues (and health costs) are higher for low income people than for higher income people. We know that there is a higher rate of health issues with native americans and other non-white groups than there is with Caucasians. We know that there is a direct relationship between food insecurity & chronic disease. Our goal is to improve the dietary eating behaviors of our guests who live at or below the poverty level by providing healthier meals. 4. Describe how the proposed project satisfies one or more of the priority funding areas identified by the Gila River Indian Community. We believe our request falls in the priority areas of Health Care. Our food programs focus on the intersections between food insecurity and health. Food insecurity has been linked with adverse diet-related health outcomes in family members of all ages including children. Statistics show that those families living in poverty have a higher rate of medical expenses than those with higher incomes. This is often due to the price variations between healthier and less Gila River Indian Community Grant Application Grant Cycle 2017 healthy foods. Our request is for $20,000 to supplement the acquisition of healthier foods. While we receive many donations of food, we often receive more foods that are high carbs like bread, pastries and desserts and fewer fruits, vegetables and protein 5. Identify the needs/problems to be addressed, target population and number of people to be served by the project. Paz de Cristo serves the poor and hungry in the East Valley. We average over 7,000 visits from 2,400 unique individuals per month from people looking for food, clothing and supportive services. We distribute an average of 300 evening meals each day. About 55% of our evening meal guests are homeless and the majority are unemployed. While most of these guests are single adults, there are many families that participate in our evening meal service. Our food box distribution averages about 1,300 food boxes per month. These recipients are mostly working poor, unemployed, single parent households and senior citizens. Our supportive services help our guests engage in the broader community and include vouchers for Arizona I.D.’s and birth certificates, bus passes for medical appointments, showers, haircuts, as well as employment assistance. Demographics: • White: 55% • Hispanic: 29% • African American: 9% • Native American: 5% • Asian 2% 6. Define the project as a new or continuing program. This is an ongoing project. 7. Provide a brief timeline including start and finish dates. Indicate if the timeline is flexible. We have been serving meals every night of the week at our present location since 1993. 8. Identify other organizations, partners or funders participating in the project and their roles. The United Food Bank distributes food, Lutheran Social Services provides overnight housing for our women guests, Mesa Municipal Courts hold a monthly homeless court at Paz, 20 different organizations provide volunteers for our evening meal service at least once a month, Circle the City provides their mobile medical clinic weekly, Community Bridges provides a Navigator and the VA provides staff members for our veterans guests. Gila River Indian Community Grant Application Grant Cycle 2017 9. Would you be able to implement the proposed plan if your organization received partial funding for this project? Yes. 10. Describe your plan for project sustainability beyond the grant period. If this is a program/project previously funded by the Gila River Indian Community describe efforts made towards the previously described sustainability plan. We have a varied source of funding including individual donations, fund raising events and grant requests. 11. Describe your plan to document progress and results. We track all of our guest visits and program activities through a software product from SoxBox. 12. Indicate any application to and/or awards made by a tribe other than the Gila River Indian Community for state shared revenues for this and any other project for the past five (5) years. If this information is included in a separate attachment, please indicate that here. We have had an award of $15,000 from the Tohono O’odham Nation for our food program last year.
