AMENDMENT TO AFFILIATION AGREEMENT
BETWEEN
MARICOPA COUNTY, by and through its DEPARTMENT OF PUBLIC HEALTH
AND
THE UNIVERSITY OF ST. FRANCIS
For The Rotation of Students
I. The above named agreement is hereby amended as specified below.
A. The preamble shall be amended as follows: "This Off-Site preceptor Student
Rotation Training Agreement ("Agreement") sets forth the terms of the rotation of
Students from the University of St. Francis ('USE'), a non-profit corporation by
and through its College of Nursing, with its principal office located at 500 Wilcox
St., Joliet, IL 60435 to Maricopa County, by and through its Department of Public
Health ("MCDPH"), whose principal office is located at 4041 N. Central Ave., Ste.
1400, Phoenix, AZ 85012 and by and through its Department of Correctional
Health, whose principal office is located at 3250 W. Lower Buckeye Rd.,
Ste. 2100, Phoenix, AZ 85009 for educational training and experience..."
B. MCDPH shall serve as the abbreviation for both the Departments of Public
Health and Correctional Health throughout the Agreement.
C. The term of the Agreement shall remain from July 1, 2014 through June 30,
2019, in accordance with Amendment 1 to the Affiliation Agreement.
All other terms and conditions of the original Agreement shall remain in full force and
effect.
