Contract No. C-22-15-063-3-08 Amendment No. 8 AMENDMENT NO. 8 TO THE CONTRACT BETWEEN MARICOPA COUNTY ADMINISTERED BY ITS HUMAN SERVICES DEPARTMENT AND LODESTAR DAY RESOURCE CENTER Maricopa County (“County”) by and through the Maricopa County Human Services Department entered into a financial contract (“Contract”) with Lodestar Day Resource Center (“LDRC”) on or about April 15, 2015, for the provision of homeless emergency shelter and coordination of social services. The County and LDRC may be referred to individually as “Party” or collectively as “Parties.” The Parties executed Amendment No. 1 on or about June 24, 2015, Amendment No. 2 on or about October 21, 2015, Amendment No. 3 on or about December 9, 2015, Amendment No. 4 on or about June 22, 2016, Amendment No. 5 on or about August 17, 2016, Amendment No. 6 on or about September 7, 2016 and Amendment No. 7 on or about December 7, 2016. The Parties wish to amend the Agreement as follows: I. Pursuant to Section 3.0 Life of the Contract: a. The term of the Amendment shall be extended from February 28, 2017 to June 30, 2017. b. Amendment No. 8 shall be effective upon approval and execution by the Parties. II. Pursuant to Section 5.0 Funding a. Increase funding amount by $12,830. The Contract amount shall be increased from $278,383 to $291,213. III. Notwithstanding any prior Amendment to the contrary, amend the Contract to permit unexpended authorized funding for the period of July 1, 2016 until February 28, 2017 to be carried forward into the extended contract period from February 28, 2017 to June 30, 2017. IV. Parties agree that the statutory language of A.R.S. §38-511 are applicable to this Amendment and incorporate the same by reference. V. The foregoing paragraphs contain all the changes to the Contract as amended. All other terms of the original Contract and Amendments not amended hereby, remain in full force and effect as approved and amended.
