Except as provided herein, all terms and conditions of the Grant/Agreement referenced in 1, above, remain unchanged and in full
force and effect.
9. ADDITIONAL SPACE FOR DESCRIPTION OF MODIFICATION (add additional pages as needed):
Revise Paragraph III. COUNTY SHALL to include:
Revise paragraph to read: F. Designate the entire length of the Cave Creek District portion of the Maricopa Trail for use by bycicles,
hikers and pack/ saddle livestock; post as: Prohibited to Motor Vehicles”.
G. Provide a contractor to: construct a 15.5 mile trail using existing power line maintenance roads, social trails and new trails.
The project also includes the re-construction of an existing bridge over Sycamore Creek whose deck was destroyed by fire. More
detailed description is attached in Mod 1 Scope of Work and attached Map.
H. Coordinate trail maintenace with Forest Service personnel to Forest Service trail maintenance standards.
Revise Paragraph IV. The U.S.FOREST SERVICE SHALL to include:
H. Provide oversight and approve placement of interpretive and trail signage.
I. Coordinate trail maintenance with County personnel and volunteers.

2. Enter the cooperator’s agreement number, if applicable.
3. Enter the number for this Modification, i.e. 01, 02, or 03. The first modification to an instrument is ‘01’, subsequent
modifications receive a subsequent modification number (for example, the fourth modification is ’04’).
4. Enter the address of the G&A Specialist/Signatory Official responsible for this agreement.
5. Enter the address of the U.S. Forest Service Program/Project Manager or Lead Scientist responsible for this agreement.
6. Enter the cooperator’s address.
7. Enter the cooperator’s HHS Sub-Account numbers, if funding is provided on this modification (for example: G2412345003)
