Effective: 03/01/2016
1. Pursuant to: Page 4, paragraph 3.0: Right to Extend Agreement, the County has the right to extend this Agreement
for a period not to exceed five (5) years from initial agreement period. The County, therefore, is extending the
Agreement term to remain in effect for the period of 3/1/2017 through 2/28/2019.
2. The Contractor shall continue to provide the following services in accordance with the Work Statements and
Compensation Rates in effect at the time service(s) is/are rendered:
• Outpatient Ambulatory Medical Care
• Mental Health and Substance Abuse Services
• Oral Health Services
• Case Management Services
• Early Intervention Services
• Treatment Adherence Services
• Health Education and Risk Reduction
ALL OTHER PROVISIONS OF THIS AGREEMENT REMAIN UNCHANGED.
