SERIAL 171046 S MEDICAL LABORATORY SERVICES DATE OF LAST REVISION: January 11, 2017 CONTRACT END DATE: January 31, 2019 CONTRACT PERIOD THROUGH JANUARY 31, 2019 TO: All Departments FROM: Office of Procurement Services SUBJECT: Contract for MEDICAL LABORATORY SERVICES Attached to this letter is published an effective purchasing contract for products and/or services to be supplied to Maricopa County activities as awarded by Maricopa County on January 11, 2017 (Eff. NTP). All purchases of products and/or services listed on the attached pages of this letter are to be obtained from the vendor holding the contract. Individuals are responsible to the vendor for purchases made outside of contracts. The contract period is indicated above. AS/mm Attach Copy to: Office of Procurement Services Melba Davidson, Correctional Health Services Dr. Jeffery Alvarez, Correctional Health Services (Please remove Serial 16040-RFP/09088-S from your contract notebooks) SERIAL 171046-S SERIAL 171046-S Garcia Clinical Laboratory Responses to Questions, IFB Serial 171046-S 01-02 Medical Laboratory Services Question 1) Demonstrate your experience of at least five (5) years prior hospital laboratory and/or correctional health laboratory management experience of similar size and population as the Maricopa County Jail and Detention facilities which houses a daily population of approximately 8,200 within a complex of 6 county jails containing 9 patient care service areas. Refer to pages 7-8 of IFB 171046-S, which detail the Maricopa County jail system and approximate population size. Garcia currently provides lab services to over 600 jails and prisons throughout the United States. Included in this number are several complete State Prison Systems. For example, the State of Michigan, with approximately 45,000 inmates in 33 locations. Just in the Jackson, Michigan area, there is a similar situation to Maricopa County. There are 5 Prison locations, including an Intake Facility and a Health Center with 112 Inpatient beds. Local Inmate population among all prisons in the area is approximately 8000. We pick up specimens, provide testing and report results into an EMR. We provide for Stat services, both in-house and at a local hospital. Garcia has been providing lab services to the entire State of Michigan since 1998, including these Prisons in the Jackson area. For the last four years, we had provided lab services to the Arizona Department of Corrections for their State operated facilities (35,000 Inmates). These sites are similar in scope to the Maricopa County situation. For example, ASPC Lewis has 16 units with a population around 6000. This is just one of the Arizona State Prison sites (there are 10 of them), each with multiple locations at the site. We had the specimens picked up by courier at the State sites and had the specimens transported to our facility for testing. Orders were placed and Results were sent to the State’s EMR electronically. Stats were provided locally. We provide the same services to the Department of Corrections for the States of Virginia (17 locations), Maryland (33 locations), Pennsylvania (27 locations), Tennessee (13 locations), as well as numerous Jails. We provided lab services to the Allegheny County Jail (population of 2250) for 10 years. We have previously provided these same services to the State Prison Systems in the States of Missouri, Arkansas, Indiana, Wyoming, Idaho and Minnesota. We have been providing lab services to Jail and Prison sites since 1998. We feel that our experience in providing laboratory services to these entities provides us with the experience needed to be able to serve the needs of the Maricopa County Jail. Question 2.a. and 2.b.) Demonstrate your plan to provide complete compatibility within thirty (30) days of award with the existing CHS’s TechCare system. Your response of “site visits by Garcia representatives to introduce Garcia to CHS employees, train them in the use of Garcia’s online system and the supplies provided” does not address how you will interface with the existing CHS TechCare System. We have been in contact with your EMR provider and have secured a copy of their specs for Orders and Results. Our computer programmer has reviewed these specs and has advised that there is nothing in the specs that would cause a problem. We have developed interfaces with EMR’s, including the eOMIS EMR with the State of Arizona, the NextGen EMR which is in both Michigan and Maryland, CorEMR, being used by several of our clients around the country, and Sapphire, which is being used by the State of Pennsylvania. Our main repository for results, and for lab test ordering, is via CareEvolve, an on line ordering and reporting system involving a bi-directional interface. We hope that this demonstrates that we have experience with creating and utilizing interfaces. As for our plan to complete the interface within the 30 day period, we are already starting the process by working on items on our end that would speed up the development of the interface. It is anticipated that if we are awarded the contract, we would be able to hit the ground with at least some of the work already completed. We have had every indication from the staff at TechCare that we will be able to work with them in this development. Because everything utilizes HL7 formatting, we will need to make some adjustments to the programming to make sure it is compatible with TechCare, which, with TechCare’s cooperation, we will be able to have implemented and tested and in place within the 30 day period. Jeff Peterson will be the main contact person in this project, and our Chief SERIAL 171046-S Programmer, Michael Brown, will be responsible for constructing the interface. We have already cleared the decks and are prepared to work on the interface solely until completion. We will still be scheduling on site visits by our staff to review the changeover from the current lab vendor to Garcia; included in that will be how to utilize our on line system. We have already undertaken significant analysis of the TechCare system and can state with high confidence that we will complete all interface requirements within the 30 day time period. Question 3) Please clarify license credentials of the Director of Clinical (Pathology) Laboratory to demonstrate accordance with the Arizona Revised Statues. Our Director, Dr. Lorenz P. Kielhorn, MD, is Board Certified in Anatomic and Clinical Pathology, copies of his License and Certification have been provided. Under A.R.S. Title 32, Chapter 13, Article 2, specifically 32- 1421.B.1, Exemptions from licensing requirements, Dr. Kielhorn is a Doctor of Medicine residing in another jurisdiction who is authorized to practice medicine in that jurisdiction, and as he engages in actual single or infrequent consultation with a doctor of medicine licensed in the State of Arizona, he is exempt from the licensing requirements (copy attached). Question 4.a.) Specified timeframes of test results are a requirement of the work as referenced in IFB Sections 2.5.11, 2.4.5.6.1 and 2.4.5.7 (state immediately above) have not been addressed by your bid response. Please clarify to demonstrate ability to meet these specification requirements, including method and equipment used. Including reporting of “critical values” or abnormal test results and timely reporting of positive tests for communicable diseases. Garcia will test and report Maricopa County results within 24 hours of arrival at the laboratory. The timetable is as follows: a. Pick up specimens approximately 5 pm Arizona time. These will be packaged in special shipping packages provided by Garcia. b. Transport specimens to airport/collection site. This will be by courier/UPS. c. Specimens transported overnight to the laboratory, arriving approximately 6-7 am Arizona time the following morning at the laboratory. d. General testing proceeds during the day and is sent to the Maricopa EMR electronically when completed, by 5 pm Arizona time. This is within the required 24 hours of receipt at the laboratory. The equipment used is referenced in the answer to Question number 8 below. e. Specimens that require additional testing time (esoteric testing, cultures) will be reported upon completion. Reference Laboratory testing will be reported within 10 days of arrival at the laboratory unless the testing requires a longer time period to accomplish. f. Gynecological testing will be reported within 5 days of arrival at the lab. Non-gynecological Cytology testing will be reported within 2 days of arrival at the laboratory. Critical results will be reported to the facility immediately, via phone. Additionally, our protocol calls for speaking directly with nursing staff, recording date and time, having staff read result back, and faxing the result to the facility. This is all noted at the laboratory for future reference as well as on the actual result report. Positive communicable disease results will be faxed to the facility within 6 hours of availability of the positive results to Infection Control. Calls will also be placed to Infection Control within this time period. Results are uploaded to our web site throughout the day and are easily accessible to personnel with credentials. We will provide these credentials to staff indicated by CHS. The results are reviewable and printable at our web site from any computer/device with web access. SERIAL 171046-S Question 5) Demonstrate that your quality assurance program meets specification requirements relative to timeframes on stat results. We contract with St. Lukes Hospital in Phoenix to provide Stat testing. We contract with Intelliquick Courier to pick up the specimens and transport them to the hospital. Intelliquick has varying levels of service available; we require that they place our calls on high priority and that they arrive at the facility within 2 hours for pickup and transportation to the hospital lab. Our agreement with St. Lukes has been to provide Stat test results to the facility within 2 hours. St. Lukes is also required to provide Critical (Panic) results via phone to the ordering facility. This is followed up with a fax of the results to the facility. This will be done within at least one hour of the availability of the results. We will provide a Quality Assurance Program to specifically monitor results coming from St. Lukes. This program is outlined below: 1. Requirement: Pickup within 2 hours of call. Monitor: review tickets for time of call vs. time arrived. Additionally, review for time delivered to destination. Note outliers (greater than 2 hours to arrival at hospital lab), communicate results to hospital lab on at least a monthly basis, more often if needed. Followup: Require a corrective plan of action from courier in the event of consistent failure to meet goals. 2. Requirement: Testing and reporting of results. Monitor: review results reports for reporting time. Compare to delivery time on courier ticket. Note outliers (greater than 2 hours from time of delivery to time of report). Communicate results of the monitor to the hospital lab at least monthly, more often if needed. Followup: Require a corrective plan of action in the event of consistent failure to meet goal(s). Question 6) Submission of an equipment list as required in IFB Section 2.5.3.6 specification is not evident in your bid response. Provide clarification. We will provide Dell Inspiron One 2020 All-in-One computers to each of the 9 locations at Maricopa County. In the event of expansion, we will provide the same to new additional sites. We will provide HP P-2035 printers for each of the computers, as well as cables to connect them. The computers will come with software necessary to connect to the internet to make use of web based programming (such as Garcia’s on line ordering and reporting system). The combination of these two devices will allow users to order testing and print results via your EMR (with the addition of any programming that your EMR may require). List: Dell Inspiron One 2020 Computer, running Windows 7 or greater. HP P-2035 Printer Toner for HP P-2035 Printer Printer Cable The centrifuges we provide are Drucker Model 642E’s. Client Services has a form that they must follow, which includes noting who the information was relayed to, the actual result, the date and time. We require that the person on site read back to us the result that was provided. The file (record) is noted with this information and becomes part of the printed report. Additionally, we have the ability to email results to individuals (practitioner, for example) and/or supervisors, in any number. So, for example, if CHS requests, a list can be made of recipients that would be included on an email of the Panic, or Critical, result. We currently provide this service to some clients who wish several layers of supervisors be made aware of Panic, or Critical, results. This happens automatically. Normal test values are reported on the individual reports as they come back to the EMR. 5) Results become available throughout the day. Most General testing is done and reported by 5 pm Eastern Time. In the event of re-runs, re-checks, etc., the results may be completed and verified throughout the night, with the majority completed by 2 am Eastern Time. These results will be transmitted to the facilities through the TechCare EMR. All our results are also uploaded throughout the day and night to our website and are available for review/printing. CHS staff will be provided with credentials so that they may access this data at any time. In the event the facilities so desire, we can also autofax results when they are completed. These would be faxed directly to a fax machine at the facility where the patient is located. 6) The results will be sent in HL7 format to CHS’ EMR, TechCare. Copies of two (2) sample HL7 reports are attached. Also, examples of hard copy reports from our on line system are attached. On this report, you may choose to have cumulative reporting (current plus previous 4 results reported) or not. Copies from our LIS are also attached. This would be the format for a faxed report. Question 9) Local customer service representation is not evident in your bid response. Please clarify how you will meet this specification requirement.
