Central Ave, Suite 600 Phoenix, AZ 85012 Dear Dr. White, Congratulations, the Maricopa County Department of Health has been selected to participate in CSTE’s 2016/2017 “State, Local, Tribal and Territorial Grants for Novel Surveillance in Behavioral Health” to pilot the collection, analysis and reporting of the CSTE Recommended Substance Abuse and Mental Health Indicators. All funding for this award shall be contingent upon the availability of funds from CDC (Cooperative Agreement No. U39OT00143-04). A contract between CSTE and Maricopa County Department of Health will be initiated for the project period. Your award will be a fixed price contract in the amount of $14,883.00 and the project period will begin February 15, 2017 and will end June 30, 2017.
