Contract No. C-22-15-063-3-07 Amendment No. 7 AMENDMENT NO. 7 TO THE CONTRACT BETWEEN MARICOPA COUNTY ADMINISTERED BY ITS HUMAN SERVICES DEPARTMENT AND LODESTAR DAY RESOURCE CENTER Maricopa County (“County”) by and through the Maricopa County Human Services Department and Lodestar Day Resource Center (“LDRC”) entered into a financial Contract on or about April 15, 2015, for the provision of homeless emergency shelter and coordination of social services. The County and LDRC may be referred to individually as “Party” or collectively as “Parties.” The Parties executed Amendment No. 1 on or about June 24, 2015, Amendment No. 2 on or about October 21, 2015, Amendment No. 3 on or about December 9, 2015, Amendment 4 on or about June 22, 2016, Amendment 5 on or about August 17, 2016 and Amendment 6 on or about September 21, 2016. The Parties wish to amend the Agreement as follows: I. Pursuant to paragraph 4 of the Agreement, amend Paragraph 3.1 extending the expiration date from October 31, 2016 to February 28, 2017, effective October 31, 2016. II. Amend Paragraph 5.0 a (ii) which identifies funding sources as defined by Catalog of Federal Domestic Assistance (CFDA) and revise funding source 14.218, Community Development Block Grant (CDBG) from $76,475 to $115,000 for Program Year 2016. III. Notwithstanding any prior Amendment to the contrary, amend the Agreement to permit unexpended authorized funding for the period of July 1, 2016 until October 31, 2016 to be to be carried forward into the extended contract period from October 31, 2016 to February 28, 2017. IV. Parties agree that the statutory language of A.R.S. §38-511 are applicable to this Amendment and incorporate the same by reference. V. The foregoing paragraphs contain all the changes to the Agreement as amended. All other terms of the original Agreement and Amendments not amended hereby, remain in full force and effect as approved and amended.
