AMENDMENT TO INTERGOVERNMENTAL LETTER OF AGREEMENT
BETWEEN MARICOPA COUNTY
By and Through DEPARTMENT OF PUBLIC HEALTH
AND Maricopa County Special Health Care District dba
Maricopa Integrated Health System (MIHS)
1. The above named Agreement is hereby amended in accordance with Paragraph 4. TERM AND
TERMINATION; this Agreement is being extended.
a. Amend Paragraph 4. TERM AND TERMINATION as described below.
b. The initial term of this Agreement is from January 7, 2016 through January 6, 2017.
2. The effective date of this Amendment shall be January 7, 2017 through January 6, 2019, with
the option of an additional extension of two (2) years.
3. All other terms and conditions of the original Agreement and its amendments shall remain in full
force and effect.
