HUMAN SERVICES DEPARTMENT

Amended Contract Termination Date: December 31, 2017

This Amendment is entered into by and between the Housing Authority of Maricopa County,
(hereinafter referred to as “HAMC”) and Maricopa County, administered by its Human Services
Department, (hereinafter referred to as “Contractor”). HAMC and the Contractor are collectively
referred to herein as the “Parties” and individually as a “Party.”
Whereas, the Parties agree the purpose of this Amendment is to extend the Agreement for a twelve-
month period as authorized in Section 11 of the Agreement. One twelve-month extension period
remain in the Agreement.
Therefore, with consideration mutually acknowledged by both Parties, amend the Agreement as
follows:
A. Extend the contract expiration date from December 31, 2016 to December 31, 2017.
B. Increase the contract award by $69,000 from $ $103,212 to $172,212.
C. Include a revised Attachment B, Itemized Service Budget 2016 FSS Grant, attached hereto
and incorporated herein.
D. This Amendment is subject to A.R.S. §38-5111.
E. The Parties to this Agreement have chosen attorneys employed by the Civil Services Division
of the Maricopa County Attorney’s Office (“Civil Services Division”) to act as their attorney for
the purpose of reviewing this Agreement. In so doing, by their signature below, each party
acknowledges that it is aware that the Civil Services Division will represent both parties to this
Agreement, and each party expressly waives any conflict created thereby.
F. The foregoing paragraphs contain all the changes made by this Amendment.
G. This Amendment is subject to A.R.S. §38-511.
H. Except as amended herein, all other terms and conditions of the Original Agreement remain
in full force and effect.
