C-50-15-052-M-02 Amendment No. 2 AMENDMENT TO THE AGREEMENT BETWEEN MARICOPA COUNTY BY AND THROUGH THE MARICOPA COUNTY SHERIFF’S OFFICE AND KEOGH HEALTH CONNECTION (HEALTH CARE EDUCATION AND SERVICES) I. Maricopa County by and through the Maricopa County Sheriff’s Office entered into a one- year renewable Agreement (C-50-15-052-M-00) with Keogh Health Connection on February 18, 2015, retroactively effective to November 1, 2014 through November 1, 2015. On October 21, 2015, the Board approved Amendment No. 1 and the parties extended the terms of the Agreement until November 1, 2016. By this Amendment No. 2, the parties wish to extend the terms of the Agreement until November 1, 2017. II. Therefore, Section 1 of the Agreement is amended extending the expiration date of the agreement from November 1, 2016 to November 1, 2017. This agreement is subject to A.R.S. §38-511. III. The foregoing paragraphs contain all the changes to the Amendment. All other terms of the original agreement not amended hereby, remain in full force and effect.
