Contract No. C-22-15-063-3-06 Amendment No. 6 AMENDMENT NO. 6 TO THE CONTRACT BETWEEN MARICOPA COUNTY ADMINISTERED BY ITS HUMAN SERVICES DEPARTMENT AND LODESTAR DAY RESOURCE CENTER Maricopa County (“County”) by and through the Maricopa County Human Services Department and Lodestar Day Resource Center (“LDRC”) entered into a financial Contract on or about April 15, 2015, for the provision of homeless emergency shelter and coordination of social services. The County and LDRC may be referred to individually as “Party” or collectively as “Parties.” The Parties executed Amendment No. 1 on or about June 24, 2015, Amendment No. 2 on or about October 21, 2015, Amendment No. 3 on or about December 9, 2015, Amendment 4 on or about June 22, 2016, and Amendment No. 5 on or about August 17, 2016. The Parties wish to amend the Agreement as follows: I. Amend Paragraph 8.0 Responsibilities of Organizations: remove in its entirety and replace with the following: 8.0 RESPONSIBILITIES OF ORGANIZATIONS 8.1 Beginning September 1, 2016 LDRC will be responsible for: 8.1.1 Staffing. Providing adequate staffing for Human Services Campus (HSC) baggage check service to align with overflow shelter hours of operation 8.1.1.1 LDRC will provide storage for clients’ personal possessions at the Campus baggage check area. 8.1.1.2 LDRC will maintain a Campus bag and tag storage for clients. 8.1.1.3 Service will be open daily from 6:00 AM to 9:00 AM in the morning and from 5:00 PM to 8:00 PM in the evening. 8.1.2 Identification. To meet the service coordination goals, LDRC will check clients into the Homeless Management Information System (HMIS). HMIS will be used to ensure all clients identified. II. Reduce the budget for the period of July 1, 2016 to October 31, 2016 from $376,476 to $278,383. III. Substitute the revised Payment Claim Form in Attachment B as part of this Amendment. IV. The Amendment shall be effective upon approval and signature by both parties. V. The foregoing paragraphs contain all the changes to the Agreement as amended. All other terms of the original Agreement and Amendments not amended hereby, remain in full force and effect as approved and amended. VI.
