Attachment I
Flu Near You Data Implementation Pilot Project
1. Name: Maricopa County Department of Public Health
2. Method of Selection: Competitive Bid
3. Period of Performance: August 1, 2016 – May 30, 2017
4. Scope of Work
Background
Flu Near You (FNY) is a participatory disease surveillance system for volunteer reports of ILI
symptoms that was created in 2011 through a collaboration between the American Public
Health Association (APHA), HealthMap of Boston Children’s Hospital, and the Skoll Global
Threats Fund (SGTF). FNY maintains a website (www.flunearyou.org) and mobile application
that allows volunteers in the United States and Canada to report their health information using a
brief weekly survey. FNY allows individuals to register using its website, a mobile application, or
Facebook. The system collects symptom data on a weekly basis, which it publishes to the
website, while offering an interface to compare its data with data from the Centers for Disease
Control and Prevention (CDC) sentinel influenza network in the form of maps and charts.
Having completed its fourth season in 2015-16, Flu Near You has demonstrated strong
correlation with the Centers for Disease Control’s ILINet surveillance system (Smolinski et al,
2015). FNY also hosts a “Vaccine Finder” tool that allows individuals to identify local sources for
influenza vaccine and vaccines for other diseases. Any resident of the United States or Canada
who is at least 13 years old can register to participate in FNY.
The mission of FNY is to collect and freely share participant-reported ILI data that are free,
open, and timely, with the general public and key stakeholders (departments of public health,
other government agencies, health care providers, etc.) to increase awareness and insights
about ILI activity. The long-term vision for FNY is to generate the earliest signal of ILI
occurrence and provide user-friendly and actionable insights to the general public and key
stakeholders. The system also aims to comprehensively track ILI symptoms across the United
States.
SGTF and CSTE have partnered to enable access to, and use of, Flu Near You data for state
and local health agencies. The purpose of this pilot project is to encourage and facilitate the use
of Flu Near You data, by enabling a sample of U.S. health departments to acquire, manage,
analyze and interpret data from the Flu Near You participatory surveillance system in order to
augment existing influenza surveillance methods.
Objectives
The objectives of the Flu Near You Data Implementation Pilot Project are to:
1. Increase awareness and understanding about potential uses of Flu Near You as a data
source among state, local, and territorial epidemiologists.
2. Assess the utility of Flu Near You data as a novel data source for influenza surveillance
at the state and local level.

3. Expand public awareness of Flu Near You and increase participation within pilot
jurisdictions.
4. Develop new analytic tools for utilization of Flu Near You data.
Methods
Maricopa County Department of Public Health will use funding primarily to implement content
delivery in alignment with the proposed project to address all of the project deliverables, in
collaboration with CSTE and SGTF.
To increase awareness and understanding about potential uses of Flu Near
You as a data source among state, local, and territorial epidemiologists, Maricopa County
Department of Public Health (MCDPH) will:
1. Prepare a brief report of Maricopa County FNY data available during the 2015-2016
season. The report will provide descriptive statistics of recent participation, user
demographics, and trends of ILI during the most recent influenza season. The brief
report will be presented to approx. 50 employees. Presenters will challenge employees
to become sustaining contributors to the application and request that at least 10
employees commit to participate >85% of the weeks during the 2016-2017 season. At 1
month and 8 months following the initial presentation, attendees will be asked to
complete a survey that measures perceptions about FNY system attributes (e.g.,
simplicity and acceptability; standard surveillance evaluation measures outlined by CDC)
2. Create a SAS program to be used for analyzing data from previous seasons and
monitoring real-time FNY ILI data during the 2016-2017 season. Weekly reports of
county FNY data will be emailed to the Office of Epidemiology email list to encourage
use.Include FNY data in weekly influenza surveillance reports. The report will be sent to
LHDs by email every Thursday and posted to the external website during the influenza
season. LHDs will further disseminate the report to their community partners.
3. Prepare an in-depth report of county FNY data available from 2012 – 2017 (5 influenza
seasons) to be presented at a department-wide meeting.
To assess the utility of Flu Near You data as a novel data source for influenza surveillance at
the state and local level, MCDPH will:
4. Use similar methods to those described by the FNY team (Smolinski et al 2015) to
determine whether FNY ILI correlates with existing influenza data in Maricopa County.
FNY will be compared to data from CDC ILINet, hospital discharge data, confirmed
positive laboratory reports (mandated), NSSP BioSense, and the Arizona Prehospital
Information and EMS Registry System.
To expand public awareness of Flu Near You and increase participation within
pilot jurisdictions, MCDPH will:
5. Launch three campaigns to increase recruitment and retention in Maricopa County to
improve and maintain FNY data quality. Two staff, including a Public Information Officer
and a Web Specialist from the MCDPH Office of Communication Engagement, will serve
as subject matter experts on public information and community engagement (both listed
Page 2 of 5

in the “Contact information” section). Each campaign will be evaluated with appropriate
process measures (e.g., number of social media messages posted, number of
promotional items distributed, and number of clinic visitors enrolled on-site) and outcome
measures (e.g., proportion of each youth organization reporting > 85% of the weeks,
number of youth reporting > 60% of the weeks, and percent increase in FNY reporting
after launching each campaign).
a. Social Media Campaign: MCDPH will work with communications experts to
create messages that aim to raise community awareness about participatory
surveillance, increase FNY enrollment, and encourage regular reporting within
Maricopa County zip codes. The messages will be posted to the MCDPH Twitter
feed (over 4,000 followers as of July 2016), Facebook page (over 1,700 page
likes), and YouTube channel ( over 300 subscribers).
b. Youth recruitment campaign: MCDPH will leverage existing leadership,
programmatic, and communication infrastructure to increase interest among
Maricopa County’s younger residents. MCDPH will host an introductory meeting
at the beginning of the season to discuss collaborative projects with the youth
organization leadership. Projects will be aimed at encouraging youth to become
“citizen scientists” by actively engaging with the FNY platform and, more
generally, public health surveillance through digital disease detection. Youth will
be provided with educational and promotional FNY material. A meeting will be
held at the end of the season to share findings from the projects, discuss lessons
learned, and award winners.
c. Community Engagement Campaign: MCDPH will reach out to public health
volunteer organizations (e.g., University of Arizona Student Aid for Field
Epidemiology Response (SAFER) Program and Medical Reserve Corps) to
educate the groups on the FNY application and request their assistance in
promoting FNY at influenza vaccine clinics in Maricopa County. During FNY
promotional days at the clinic, volunteers will set up a free-standing informational
display, hand-out recruitment cards, and help people enroll on the application. A
meeting will be held at the end of the year to discuss lessons learned, successful
campaign strategies, and recommendations for promoting the application during
future seasons.
To develop new analytic tools for utilization of Flu Near You data, MCDPH will:
6. Determine FNY’s potential for monitoring non-ILI syndromes such as acute respiratory
illness (ARI), respiratory syncytial virus (RSV), and fever+rash (possible measles). Non-
ILI FNY data will be compared to syndromic surveillance data from NSSP – BioSense
Platform to look for correlations and biases within each data source.
