MIHS INFECTIOUS DISEASE
It is mutually agreed that the Contract referenced is amended as follows:
1. Pursuant to the Terms and Conditions, Section Eight (8), Contract Extension, the term of the agreement is hereby
extended to April 27, 2018 by this Amendment One (1).
2. Section Six (6), Insurance and Indemnification will be replaced with the following by this Amendment One (1).
6.2.1 shall be replaced with the following:
Each Party (as "indemnitor) agrees to indemnify, defend, and hold harmless the other Party (as "indemnitee") from
and against any and all claims, losses, liability, costs, or expenses (including reasonable attorneys fees)
(hereinafter collectively referred to as "claims") arising out of the negligent performance of this Agreement, but only
to the extent that such claims which result in vicarious/derivative liability to the indemnitee are caused by the act,
omission, negligence, misconduct, or other fault of the indemnitor, its officers, officials, agents, employees, or
volunteers.
All other provisions shall remain in their entirety.
Additional signatures are included on page Two (2) of this amendment.
Contractor hereby acknowledges receipt and acceptance of The above referenced Contract Amendment is hereby
above amendment and that a signed copy must be filed with executed this day of , 2016 at
