AMENDMENT No. 2 to LEASE AGREEMENT
Between HTA-THUNDERBIRD MEDICAL, LLC, LESSOR
And MARICOPA COUNTY, LESSEE
RECITALS
A. Lessor and Lessee are parties to that certain Lease Agreement No. L-7357 dated and approved
by the Maricopa County Board of Supervisors January 4, 2005 and subsequently amended
December 15, 2015. The Lease Agreement is for approximately 4,292 rentable square feet of
office and clinic space at 5422 W. Thunderbird Road, Glendale, AZ 85306. This space is utilized
for WIC Services.
B. The current term of the Lease Agreement expires July 31, 2016.
C. Lessor and Lessee now mutually desire to amend the Lease Agreement to extend the lease term,
adjust the lease rate, and update Lessee Notice address.
AGREEMENT
NOW THEREFORE, in consideration of the foregoing recitals and other good and valuable
consideration, receipt and sufficiency of which is hereby acknowledged, Lessor and Lessee agree
as follows:
1. The term of the Lease Agreement is hereby extended three (3) years and 2 months,
commencing August 1, 2016, and expiring September 30, 2019. This Lease Agreement is subject
to termination pursuant to the provisions of A.R.S. § 38-511, the provisions of which are
incorporated herein by this reference.
2. Lessee agrees to pay as base rent, in equal monthly installments, the sums as follows:

The above amounts are subject to rental taxes which may or may not increase.

Section 20 of the Lease Agreement is updated as follows:

3. The effective date of this amendment is the date of execution by the Maricopa County Board of
Supervisors.
4. The foregoing paragraphs contain all the changes made by this amendment. All other terms and
conditions of the original agreement and as modified by subsequent lease amendments remain
the same and in full force and effect.
