Enclosed are the documents for reimbursement regarding the 2012-2013 Contract# C 86-07-
058-2-04 for the Mesa Fire and Medical Immunization Program. The services performed met the
requirements of the contract that was signed by Mesa Fire and Medical Department. The request
was previously submitted for payment on May 30, 2013. It was also requested August 26, 2014
and most recently August 10, 2015.
