for
Childhood Immunizations
The above named agreement is hereby amended as specified below:
A. Coversheet, Line 3: Increase the not-to-exceed aggregate amount to $586,000,
an increase of $75,000 for the budget period June 3, 2012 to May, 31, 2013.
B. Coversheet, Lines 6: replace expiration date with May 31, 2013.
C. Revise Section IV, 2. Method of Payment, A. Administration Fee rate of $15.00
per child immunized, per visit.
II. All other terms and conditions of the original Agreement shall remain in full force and
effect.

RESOLUTION NO. 100-77
A RESOLUTION OF THE CITY COUNCIL OF THE
CITY OF MESA, MARICOPA COUNTY, ARIZONA
APPROVING AND AUTHORIZING THE CITY
MANAGER TO EXECUTE A 4 th AMENDMENT TO THE
INTERGOVERNMENTAL AGREEMENT BETWEEN
THE CITY OF MESA AND THE MARICOPA COUNTY
DEPARTMENT OF PUBLIC HEALTH (MCDPH) TO
PROVIDE CHILDHOOD IMMUNIZATIONS.
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF MESA,
MARICOPA COUNTY, AS FOLLOWS:
Section 1: That the Intergovernmental Agreement between the City of Mesa and
the Maricopa County Department of Public Health (MCDPH) to provide childhood
immunizations and vaccinations, is hereby approved.
Section 2: That the City Manager, Christopher J. Brady, or his designated
representative, is authorized and directed to execute the agreement, and the City Clerk
is authorized and directed to attest to the signature of the City Manager thereon.
PASSED AND ADOPTED by the City Council of the City of Mesa, Maricopa
County, Arizona, this 25 th day of June, 2012.
