H-FIN is subcontracted by the Town of Gila Bend and is housed in the Gila Bend Family Resource Center making it a one-stop location. HFIN staff have been trained accordingly and assist with the completion of forms for federal and state social service programs. Buckeye residents are referred to the H-FIN office in Buckeye for assistance in completion of the governmental forms. 7.0 Grievance Procedure 7.1 The Respondent shall establish a system through which applicants for, and recipients of, services may present grievances and may take appeals about eligibility and other aspects of the Respondent’s work under this Award. The grievance procedure shall include provisions for notifying the applicants for, and recipients of, services of their eligibility or ineligibility for service and their right to appeal to the Department if the grievance is not satisfied at the Respondent’s level. Clients are advised of their rights and responsibilities and grievance/problem procedure during their application. In the event a complaint is stated by the client, prior to taking an application, the Director is notified of the situation and the Director will provide the above information to the client immediately. A denial/approval letter is then issued to all financial assistance applications. 8.0 Pandemic Award Performance 8.1 The Respondent shall provide a written plan that illustrates how the Respondent shall perform up to Award standards in the event of a pandemic. At a minimum, the pandemic performance plan shall include: 8.1.1 Key succession and performance planning if there is a sudden significant decrease in Respondent’s workforce. 8.1.2 Alternative methods to ensure services are available. 8.1.3 An up to date list of agency contacts and organizational chart. 8.2 In the event of a pandemic, as declared by the Governor of Arizona, U.S. Government or the World Health Organization, which makes performance of any term under this Award impossible or impracticable, the Department shall have the following rights: 8.2.1 After the official declaration of a pandemic, the Department may temporally void the Award in whole or specific sections if the Awardee cannot perform to the standards agreed upon in the initial terms. 8.3 The Department shall not incur any liability if a pandemic is declared, and emergency procurements are authorized by the Director of the Arizona Department of Administration per A.R.S. § 41-2537 as may be amended of the Arizona Procurement Code. 8.4 Once the pandemic is officially declared over and/or the Awardee can demonstrate the ability to perform, the Department, at its sole discretion may reinstate the temporarily voided Award. With a total of two CAP staff members, a pandemic may affect service delivery. Clients would be given a list with alternate resources to access (e.g. Salvation Army Red Cross, local churches). MCHSD would be notified immediately if service delivery was impacted significantly. 44 Section V Attachments ATTACHMENT B - OPERATING BUDGET for SFY17 Contract Operating Budget -County June 1, 2016 to June 30, 2017 CONTRACTOR: Town of Gila Bend Service: Crisis Case Management and Financial Assistance Services Revenue Sources Maricopa County* 35,000 In Kind Rent Cost 17,500 Town of Gila Bend 63,900 Total Revenues 116,400 Budget Categories Personnel / ERE 35,000 Total Expenses 35,000 *Funding will be allocated internally on a monthly basis by Maricopa County Human Services Department/Financial Services.
