AMENDMENT TO STUDENT ROTATION TRAINING AGREEMENT
BETWEEN
MARICOPA COUNTY, by and through its DEPARTMENT OF PUBLIC HEALTH
AND
CHAMBERLAIN COLLEGE OF NURSING
I. The above named agreement is hereby amended as specified below.
A. The original contract shall be amended to change the opening paragraph of the
contract to: "This Off-Site preceptor Student Rotation Training Agreement
("Agreement") sets forth the terms of the rotation of Students from Chamberlain
College of Nursing, LLC ("CHAMBERLAIN"), whose principal office is located at
3005 Highland Parkway, Downers Grove, IL 60515 and whose local office is
located at 2149 W. Dunlap Ave, Phoenix, AZ 85021 to Maricopa County
("COUNTY"), by and for its Department of Public Health, whose principal office is
located at 4041 N. Central Ave., Ste. 1400, Phoenix, AZ 85012, and its
Department of Correctional Health, whose principal office is located at 3250 W.
Lower Buckeye Rd., Ste. 2100, Phoenix, AZ 85009 for educational training and
experience. This Agreement shall commence on January 1,2015, and is valid
through December 31, 2019."
B. The abbreviation "MCDPH" shall be amended to "COUNTY" throughout the
Agreement.
All other terms and conditions of the original Agreement shall remain in full force and
effect.
