AMENDMENT No. 2
TO THE SERVICE AGREEMENT
BETWEEN
MARICOPA COUNTY
ADMINISTERED BY ITS
HUMAN SERVICES DEPARTMENT
AND
TEMPE COMMUNITY ACTION AGENCY
Maricopa County (“County”) by and through the Maricopa County Human Services Department
(“MCHSD”) and Tempe Community Action Agency (TCAA), (“Contractor”) entered into a Contract
Agreement (“Agreement”) on or about July 1, 2014 and amended the Agreement on or about June 10, 2015
for payments to landlords, mortgage companies or utilities companies on behalf of eligible clients that require
financial assistance. The County reimburses TCAA for the face value of the checks issued to vendors on
behalf of eligible clients. The Parties wish to amend the Agreement to address the following areas:
1. Extend the Agreement term from June 30, 2016 to June 30, 2017 pursuant to the terms of the
Agreement, the County may, at their option and with the approval of the Contractor, extend
the period of this Agreement up to a maximum of two (2), one (1) year options. Amendment
No. 2 is the second 1-year renewal term.
2. Amendment No. 2 shall be effective on July 1, 2016.
3. This agreement is subject to and incorporates by reference the statutory language of A.R.S.
§38-511.
4. The foregoing paragraphs contain all the changes to the Agreement. All other terms and
conditions of the original Agreement as amended remain the same and are in full force and
effect as approved and amended.
