AMENDMENT
TO THE AGREEMENT
BETWEEN
MARICOPA COUNTY
ADMINISTERED BY ITS
HUMAN SERVICES DEPARTMENT
AND
lEMPE COMMUNITY ACTION AGENCY
Amendment to the agreement between Tempe Community Action Agency (TCAA) and Maricopa County
administered by its Human Services Department ("Maricopa County") for payments to landlords, mortgage
companies or utilities companies on behalf of eligible clients that require financial assistance. Maricopa
County shall reimburse TCAA for the face value of the checks issued to vendors on behalf of eligible clients.
TCAA was the successful proposer for a Bid Quote (PQ140018) for Check Writing Services, issued by the
Maricopa County Office of Procurement Services (OPS) on June 10, 2014. The bid quote is for the check
writing fees only.
Maticopa County is exercising the option to extend the period of the Agreement for one (1) year term.
Maricopa County may in its sole discretion renew the agreement for an additional year, (or at the County's
option, extend the contract on a month to month basis for maximum of (6) months after expiration).
The purpose of the Amendment is to address the following:
I. Renew the Agreement by extending the end date of the Agreement from June 30, 2015 to
June 30, 2016.
The effective date of this amendment is July 1,2015.
HI. The foregoing paragraphs contain all the changes to the Agreement. All other terms and
conditions of the original Agreement as amended remain the same and are in full force and
effect.
