MARICOPA COUNTY, by and through its DEPARTMENT OF PUBLIC HEALTH
AND
MAYO CLINIC ARIZONA through its MAYO SCHOOL OF GRADUATE MEDICAL EDUCATION
The above named agreement is hereby amehded as specified below.
A. The term of this Agreement is to be extended tom July 1, 2016 through June 30,
2021.
All other terms and conditions of the original Agreement shall remain in full force and
effect.
