AMENDMENT TO THE OFF-SITE PRECEPTOR STUDENT ROTATION
TRAINING AGREEMENT
BETWEEN
MARICOPA COUNTY, by and through its DEPARTMENT OF PUBLIC HEALTH
AND
ARIZONA STATE UNIVERSITY
For The Rotation of Students
I. The above named agreement is hereby amended as specified below.
A. The term of this Agreement is to be extended from July 1, 2016 through June 30,
2021.
B. Paragraph 6A shall be amended to read "ASU and MCDPH shall not unlawfully
discriminate against any individual on the basis of age, race, religion, creed, sex,
national origin, handicap, disability, sexual orientation or veteran status. To the
extent applicable to this Agreement, both parties agree to comply with all state
and federal employment laws, rules, regulations and executive orders, including
Executive Order 2009-09."
All other terms and conditions of the original Agreement shall remain in full force and
effect.
