SUBRECIPIENT AGREEMENT Amendment #1 15-AZDOHS-HSGP- 150203-01 Between The Arizona Department of Homeland Security And Maricopa County Department of Emergency Management WHEREAS, A.R.S. § 41-4254 charges the Arizona Department of Homeland Security (AZDOHS) with the responsibility of administering funds. Pursuant to Section XII of the subgrantee Agreement between the Arizona Department of Homeland Security and the subgrantee the following section of the above referenced Subgrantee Agreement is hereby amended as follows: II. TERM OF AGREEMENT, TERMINATION AND AMENDMENTS: This Agreement shall become effective on October 1, 2015 and shall terminate on September 30, 2016. SHALL BE CHANGED TO READ: This Agreement shall become effective on October 1 2015 and shall terminate on November 3 2016. All other terms of the original Subgrantee Agreement remain effective. In Witness Whereof, the parties have set their hands to this AMENDMENT as of the day and year herein indicated and agree that all parties are obligated to follow all terms and conditions of the original subgrantee agreement and are liable for all funds received by the AZDOHS. Provide a detailed justification and explanation The county purchased COOP software, the service dates for extending the period of performance. are November 4, 2015 thru November 3, 2016. In order to fully reimburse the County for the COOP software we need to extend the period of performance from 10/1/15-11/4/16. With Grant funded projects we are not allowed to go out to bid or solicit vendors until the Board of Supervisors approves and accepts the grant. Depending on the project and if you have to go out for RFP, it could be 6 months after the start of the grant period that a contact signed.
