1. Maricopa County and Office of the Attorney General entered into Contract No. AAG014-071794 for the
provision of services to disabled veterans on or about May 7, 2014. Thereafter, on July 6, 2015, the
parties amended the agreement extending the termination date, reducing units for allowing for transfer
of unexpended funds between program years 1 and 2. The parties wish to further amend the agreement
setting a new termination date, reallocating and aggregating funds from program years and additional
units to program year 3.
2. The purpose of this amendment is to set a new termination date for Contract No. AAGO14-071794, as per
Special Terms and Conditions, Item 2.4, Term of Agreement. The new contract termination date shall be
September 30, 2016.
3. The parties agree to accept the Maricopa County Cost Sheet dated March 16, 2016 (attached) This
revision shall:
3.1 Add 3 Units for Program Year 3 with an average cost of $43,500 for a Total of $130,500 and an
Administrative Fee of $6,500 each, for a Total of $19,500 making an aggregate Total of an additional
$150,000 for Program Year 3.
3.2 Allow Maricopa County to utilize any remaining funds from the $1,249,800 aggregate award for Year
1 and Year 2 to fund uncompleted projects from Year 2.
3.3 Allow Maricopa County to add projects to Program Year 3 provided they do not exceed the aggregate
total of $1,399,800 and the projects are similar in cost to the projects described in Item 3, as amended.
3.4 The total costs for this contract shall not exceed $1,399,800.
4. Maricopa County shall provide a full accounting of all funds and return any unused funds within 30 days
of contract termination.
5. This amendment is subject to and incorporates by reference the statutory language of A.R.S. §38-511.
6. All other terms and conditions of the Contract not amended hereby shall remain in full force and effect as
approved and amended.
