AMENDMENT TO INTERN CLINICAL TRAINING AGREEMENT

MARICOPA COUNTY, by and through its DEPARTMENT OF PUBLIC HEALTH

NORTHERN ARIZONA HEALTHCARE, an Arizona nonprofit corporation on behalf of
FLAGSTAFF MEDICAL CENTER, VERDE VALLEY MEDICAL CENTER, NORTHERN
ARIZONA HOMECARE
I. The above named agreement is hereby amended as specified below.
A. The term of this Agreement shall be extended from May 1, 2016 through April 30,
2021.
All other terms and conditions of the original Agreement shall remain in full force and
effect.
