provided they complete the requirements to qualify for Program Year 2016-2017 wellness premium
reduction incentives by September 30, 2016. Employees who do not complete Program Year 2016-2017
incentive requirements by September 30, 2016 deadline will stop receiving the incentive, but can earn
and receive their 2016-2017 wellness premium reduction incentive on a prospective basis.
Program Year 2016-2017 Wellness Premium Reduction Incentives:

c. Tobacco-Free Pledge
Eligibile employees must complete both the Health Assessment and Biometric Screening to qualify for a
premium reduction of $40.00 per month. Employees must complete the Tobacco-Free Pledge and

reduction if they complete a County-sponsored tobacco cessation program in Program Year 2016-2017.
By completing the tobacco cessation program during Program Year 2016-2017, the employee will also
qualify for wellness incentive points needed in order to qualify for Program Year 2017-2018 wellness
premium reduction incentives.
Program Year 2017-2018 Wellness Premium Reduction Incentives:
In order for employees enrolled in a County-sponsored medical benefits plan to qualify to receive the
wellness premium reduction incentives in Program Year 2017-2018, employees will be required to earn
80 to 100 points no later than May 31, 2017. Wellness incentive points will be earned by completing
specific activities, as listed in Table 1. Employee Benefits and Health will be authorized to specify and
clarify requirements for earning points, and may add to the list of eligible activities.
For Program Year 2017-2018, earning 100 or more points will qualify an employee for a $60 per month
premium reduction; earning 80 to 99 points will only qualify an employee for a $40 per month premium
reduction. An employee who does not earn at least 80 points by May 31, 2017 will be ineligible to
receive the wellness premium reduction incentives in Program Year 2017-2018. Employees who
indicate that they are tobacco users and who do not complete one of the alternative tobacco cessation

month premium reduction).
