I. The above named agreement is hereby amended as specified below:
A. Coversheet, Item 3, Agreement Amount is changed to an total aggregate not-to-
exceed (NTE) amount from $12,000 to $14,000 (increased by $2,000 for the
budget period November 20, 2015 through May 31, 2016).
B. Attachment A, Schools, Page 18, remove and add the revised Attachment A.
All other terms and conditions of the original Agreement and its amendments shall
remain in full force and effect.
