SRP Weatherization
Arizona Community Action Association (ACAA), a 501(c3) non-profit social and human
service association, and Maricopa County, administered by its Human Services
Department/Community Development Division (hereinafter “County” or “Contractor”) entered
into an agreement for Low Income Weatherization Implementation ("Contract") on or about May
1, 2015. The Parties wish to amend the Contract to add an additional program year with
commensurate funding for the Salt River Project Low- Income Weatherization Assistance
Program (hereinafter referred to as the “Program”) and reporting requirements.
A. The Parties agree to Amend the Contract as follows:
1. Amending the expiration term in Section 1 from March 30, 2016 to March 30, 2017,
effectively adding an additional program year, PY2016-17.
2. Amending Section 2, Scope of Service, Compensation/Taxes in paragraph M, by
a. allocating additional funds for PY2016-17 in the amount of $271,220.00
which includes $216,976.00 for Direct Services and $54,244.00 for Program
Delivery; and
b. permitting Contractor to use up to 5% of the allocation for Health and Safety
not to exceed $13,651.
3. Replace all references “Governor’s Office of Energy Policy” to “Arizona
Department of Housing” (ADOH).
4. Amending Section 2, Scope of Service, Program Reporting, Paragraph J, adding
5. Progress reports will be submitted to ACAA on August 21, 2016 and November 21,
2016. Reports shall outline: projects completed, projects to be completed, funds
expended, and funds remaining.
B. Contractor shall be prohibited from using unexpended PY 2015-16 funding for the
PY2016-17 projects.
C. Notwithstanding the foregoing, PY2016-2017 funding will not available for expenditure
until May 1, 2016.
D. This Amendment is effective when executed by all the Parties.

E. This Amendment is subject to A.R.S. §38-511.
F. Unless otherwise noted herein, all provisions of the original Contract not mended hereby
remain in full force and effect.
