The Board of Commissioners of the Housing Authority of Maricopa County convened in Formal Session at 9:00 AM
on Wednesday, November 18, 2015 at the Maricopa County Board of Supervisors Auditorium, 205 W. Jefferson,
Phoenix, Arizona with the following members present: Denny Barney, District 1; Steve Chucri, Chairman, District 2;
Andrew Kunasek, District 3; Clint Hickman, Vice Chairman, District 4; Steve Gallardo, District 5. Also present: Tom
Manos, County Manager; Michelle Wood, Legal Counsel; and Jazminda Ryan, HAMC Management Analyst.
BOARD ACTION:
H-1. RESOLUTION 15-020 FY 2015 SECTION 8 MANAGEMENT ASSESSMENT PROGRAM (SEMAP)
CERTIFICATION
Adopt Resolution 15-020 and approve the Housing Authority of Maricopa County FY 2015 Section 8
Management Assessment Program rating as a “High Performer.” There is no financial Impact associated
with the item.
HUD 24 CFR 985.101 requires Public Housing Authorities (PHA’s) to submit the HUD required SEMAP
certification from within 60 calendar days after the end of its fiscal year. The certification must be approved
by the PHA Board of Commissioners resolution and signed by the PHA Executive Director. (C-66-16-005-M-

H-2. RESOLUTION 15-021 PUBLIC HOUSING WRITE-OFF IN UNCOLLECTIBLE TENANT ACCOUNTS RECIEVABLE
Adopt Resolution No. 15-021 and approve the write off of $15,565.45 in Uncollectible Tenant Accounts
Receivable for the period ending August 31, 2015. This action is effective upon Board approval. (C-66-16-

H-3. RESOLUTION 15-022 FISCAL YEAR 2015 MANAGEMENT AND FINANCIAL REVIEW REPAYMENT
AGREEMENT
Adopt Resolution No. 15-022 and approve the Fiscal Year 2015 Management and Financial Review
Repayment Agreement between the Housing Authority of Maricopa County and the U.S. Department of
Housing and Urban Development effective October 26, 2015. HAMC must replenish reserves in the
amount of four hundred sixty-eight thousand, seven hundred eighty-one dollars ($468,781.00) to the
HAMC’s Program Reserve Account. HAMC and HUD have agreed on a repayment plan which consist of
quarterly installments of $4687.81 over a period of 25 years commencing January 1, 2016. (C-66-16-006-

H-4. RESOLUTION 15-023 SERVICES AND SUPPORTS AGREEMENT BETWEEN MERCY MARICOPA
INTEGRATED CARE AND THE HOUSING AUTHORITY OF MARICOPA COUNTY FOR THE BRIDGE TO
PERMANENCY HOUSING PROGRAM
Adopt Resolution 15-023 and approve the Housing Authority of Maricopa County to enter into the
Services and Supports Program Agreement with Mercy Maricopa Integrated Care to become a
Bridge Subsidy Program Participating Provider. This Agreement shall be effective November 1,
2015, and shall be effective for an initial term of one (1) year and will automatically renew for
additional one (1) year terms unless and until terminated in accordance with Section 6.0 of the
Agreement. HAMC will receive a maximum reimbursement of $561,542 for all costs and expenses
for the time frame of November 1, 2015 to June 30, 2016. (C-66-16-008-M-00)

Hickman
