C-22-14-009-3-01 Amendment 1 AMENDMENT TO THE SUBRECIPIENT AGREEMENT BETWEEN MARICOPA COUNTY ADMINISTERED BY ITS HUMAN SERVICES DEPARTMENT AND TRELLIS, FORMERLY KNOWN AS NEIGHBORHOOD HOUSING SERVICES OF PHOENIX, INC. Contract Amount: $66,277 Contract Start Date: July 1, 2013 Contract Termination Date: June 30, 2016 Contract Number: C-22-14-009-3-00 Program Number: DG1305 CFDA Number: 14.218, Community Development Block Grant DUNS Number: 080668072 I. Neighborhood Housing Services of Phoenix, Inc. and Maricopa County by and through its Human Services Department (“Parties”) entered in a Subrecipient Agreement with a term of July 1, 2013 through August 21, 2015. The County provided the nonprofit organization $66,277 in Community Development Block Grant funds from the U.S. Department of Housing and Urban Development (HUD). II. On March 2, 2015, Neighborhood Housing Services of Phoenix, Inc. filed an amendment to its Articles of Incorporation with the Arizona Corporation Commission changing its name to TRELLIS. The amendment was effective February 26, 2015. III. The Parties enter into this amendment for the purposes extending the expiration term, make corresponding changes the implementation schedule, and to acknowledge the name change. IV. Now therefore, the parties agree as follows: a. Amend the agreement to substitute Trellis as the named Subrecipient; b. Extend the expiration date from August 21, 2015 to June 30, 2016, retroactive to and effective on August 22, 2015 to allow Trellis additional time to complete the previously approved Work Statement. c. Extend the Implementation Schedule in Section III Work Statement, page 38 from June 30, 2015 to June 30, 2016. This agreement is subject to A.R.S. §38-511. The foregoing paragraphs, contain all the changes made by this Amendment. All other terms and conditions of the original agreement remain the same and in full force and effect.
