I. The above named contract is hereby amended as specified below:
A. Coversheet, 1. Contract amount is changed to an aggregate, not-to-exceed
(NTE) total amount from $154,651.00 to $404,651.00 (increased $250,000.00
for the budget period November 1, 2015 to December 31, 2015).
B. Coversheet, 6. Expiration Date, extend the end date to December 31, 2015.
C. Section IV, COMPENSATION, 1. COMPENSATION, add the following
paragraph:
D. CASS may charge HCH for expenses related to clinic staffing required
to provide services as specified in this contract up to a maximum of
$250,000.
D. Section IV, COMPENSATION, 2. METHOD OF PAYMENT, E. Submit
monthly invoice by hard copy or electronically to:, replace the contact
