The Department of Public Health's Health Care for the Homeless Program (FICH) provides medical and
dental care to homeless individuals from a grant received by the Federal Health Services Resource
Administration, Health and Human Services Division. The grant requires that dental care be provided
to homeless clients, The dental provider must be in very close proximity to HCH because of the lack
of transportation among homeless individuals as well as their transient lifestyle.
SINGLE SOURCE CATEGORY (Check applicable box, attach documentation or provide explanation below.)
Maricopa County Procurement Code MC1-351 states: "Single source" means an acquisition where, competition is impractical because
of standardization, warranty, or other factors as approved by the Board of Supervisors.
NOTE: The following_are unlikely to be sufficient single source justifications:
• Personal or agency preference for a product or vendor
• Agency perception that the vendor is the best qualified (this should be determined through a competitive process)
• Lack of agency planning resulting in limited time to conduct a competitive procurement
• Special incentive or deal offered (can be assessed in open and competitive solicitation)
• Agency convenience

THIS PROCURMENT IS A SINGLE SOURCE BECAUSE (attach additional page if needed):
Central Arizona Shelter Services (CASS) operates on the same campus as HCH located at 12th Avenue and Madison in
downtown Phoenix. Because of this, they are the only viable option for a provider. In addition, CASS offers many other
amenities to the population so they have developed a strong rapport with the residents located there. The entire campus
serves as a one-stop shop for many of the individuals. CASS is also open during the same hours as the HCH clinic, making it
convenient for not only the HCH clients bu the staff as well. In addition, the amount of time and effort it would take to revert all
the current patient files and records to another provider would be not only cost prohibitive, but could destroy the viability of
the current program.
The dental provider would not only need to be in close proximity to the homeless population located on the Human Services
Campus, but they would also need to be culturally sensitive to the needs of the homeless population. Many in the homeless
population are not well-groomed or bathed, which makes them less desirable in a private dental setting.
