The Department of Public Health's Health Care for the Homeless Program (HCH) provides medical and dental care and housing
assistance to homeless individuals from a grant received by the Federal Health Services Resource Administration, Health and Human
Services Division. Housing assistant is provided through a partnership with the Human Services Campus LLC using housing vouchers
and funding that is pooled from both the HCH Program and various other grants and funds received by the Human Services Campus
LLC. This funding will incorporate a medical assessment component into the housing process for homeless individuals being placed in
permanent housing through the Human Services Campus LLC Because of that, the housing program must be in very close proximity
to to the HCH Program because of the lack of transportation among homeless individuals and for continuity of patient care.
SINGLE SOURCE CATEGORY (Check applicable box, attach documentation or provide explanation below.)
Maricopa County Procurement Code MC1-351 states: "Single source" means an acquisition where, competition is impractical because
of standardization, warranty, or other factors as approved by the Board of Supervisors.
NOTE: The following are unlikely to be sufficient single source justifications:
• Personal or agency preference for a product or vendor
• Agency perception that the vendor is the best qualified (this should be determined through a competitive process)
• Lack of agency planning resulting in limited time to conduct a competitive procurement
• Special incentive or deal offered (can be assessed in open and competitive solicitation)
• Agency convenience

THIS PROCURMENT IS A SINGLE SOURCE BECAUSE (attach additional page if needed):
The HCH Program operates on the Human Services Campus located at 12th Avenue and Madison in downtown
Phoenix. Because of this, the housing program operated by the Human Services Campus LLC is the only viable
option for a provider. In addition, the Human Services Campus LLC offers many other amenities to the population
so they have developed a strong rapport with the residents located there. The entire campus serves as a one-stop
shop for many of the individuals and many of the individuals that are eligible for housing through the Human
Services Campus LLC are already patients of the HCH Program. The amount of time and effort it would take to
revert all the current patient files and records to another provider would be not only cost prohibitive, but could
destroy the patient's continuity of care.
