Both the Welcome Center and the HCH Program operate on the Human Services

process because of the close proximity of the two facilities.
3. OBJECTIVE
Homeless individuals who meet the requirements for permanent housing will receive a
medical assessment prior as part of the housing process. The medical assessment will
ensure that the individual, prior to moving into permanent housing, has access to
primary medical care, has a sufficient supply of required medications while transferring
to a primary care provider, and has needed medical equipment.
4. SCOPE OF WORK
To provide the staffing required to develop the medical assessment component of the
housing process for homeless individuals who qualify for permanent housing, including
determining where in the process a medical assessment would be most appropriate,
and the importance of the medical assessment in the decision to house an individual.
