Project Name: Homeowner Rehabilitation for Disabled Veterans
Please provide a brief description of the project submitted to the 12% SRPMIC Grant program.
Marieopa County is dedicated to providing housing services designed to address home livability, safety
concerns, and accessibility upgrades. Marieopa County Human Services Department (MCHSD) proposes
a program that helps Veterans with service-connected disabilities to live independently in a safe and
suitably adapted home and to allow residents an opportunity to stay in their homes and age comfortably in
place. This program will be implemented using the existing Marieopa County Home Improvement
Program (MCHIP) guidelines and procedures. MCHSD will leverage federal and local funds for eligible
Veterans including weatherization funds as necessary. MCHSD will provide a maximum grant amount of
$50,000/home for Veterans with service-connected disabilities including, but not limited to, the following
types of improvements: accessibility fixture retrofits in bathrooms and kitchens, doorway widening, as
well as ramp connection and ramp modification in order to increase structure mobility, stability and safety
of homes. All MCH1P improvements are made by licensed contractors. MCHSD employs trained staff
with extensive home rehabilitation experience including work with Veterans with service-connected
disabilities, and community-based partnerships in place for efficient procurement of licensed contractors.

MCHSD will coordinate community, referrals including referrals from the SRPMIC Social Services
Department. MCHSD also utilizes an online inquiry system which allows interested Veterans to quickly
complete and submit a form to inquire about services available to them. To be eligible, Veterans must
own their home in Maricopa County, be active, honorably discharged or retired members of the armed
services, have a service-connected disability, and not be able to afford the modifications needed to their
home.
Beginning Date of Project: December 1, 2015 End Date of Project: December 31, 2016
Geographic Area to be served:
All of Maricopa County including unincorporated areas.
** Attach Detailed Budget**
Amount of Total Project: $200,000 Amount of 12% Request: $200,000
(No Multi-Year Request will be accepted)
Budget: Cost:
'Home Rehabilitation (4 -5 homes) $200,000
Total Requested $200,000
Total accessibility and related improvements including the cost of the rehabilitation services provider and
all direct construction costs will result in approximately 4 to 5 homes at a maximum of $50,000/home.
Through experience with major rehabilitation, MCHSD plans to include accessibility and related
improvement costs which may include reasonable moving or relocation costs which is considered as a
direct cost, if deemed necessary and approved in advance by the County. Relocation costs are only used
for situations where the rehabilitation consultant services provider and the eligible Veteran mutually
determine that the extent of rehabilitation work required makes it infeasible for the home to remain
occupied during the rehabilitation contract.
By assessing the home's functionality with the needs of the homeowner and related to the accessibility
improvements, MCHSD may assist Veteran homeowners with other major systems in the home (if
reasonable and necessary). Examples of major systems include fixing broken appliances or air
conditioning units, repairing leaky duct work, or repairing plumbing or electrical safety issues, etc.,
allowing for energy savings and long term stability for the homeowner.
If the Amount of funding received is less than the amount needed for the project, do you have other
funds to complete the project? (Please Explain):
No. The program will not continue after May 2016. The success of this kind of program depends on
continued sources of funding available each year. Maricopa County intends to continue to seek additional
funding which will eliminate gaps in this service for Veterans. Maricopa County hopes the SRPMIC will
consider supporting this program to expand and build upon this important work.
