Project Name: Athletic and Extracurricular Activity Sponsorships and Scholarships
Please provide a brief description of the project submitted to the 12% SRPMIC Grant program.
Opportunity4Kids believes that no child should be deprived of the opportunity to engage or participate in
any school or extracurricular activity solely on the basis of their financial status. It is our mission to
ensure that children in Arizona have resources- whether that be through providing monetary relief,
equipment, unifoiiiis or sponsoring extracurricular activities.
Through the Opportunity4Kids website, parents can apply for Sponsorship/Scholarship. This online form
requests information about the child such as their age, current school, location, the activity or club they
are in, and what they need in order to participate. To prove that these families are in financial need, they
are asked a series of question in order to pre-qualify their conditions. Parents must also upload a proof of
need, which states that they are truly in need of sponsorship (i.e. tax statement, invoice, financial
documents, etc.) This information is then funneled though the Sponsorship Request process where
information and need are verified. In this confirmation process, we give priority to those individuals with
the most need, meaning those with the lowest income.

With the requested Grant Funding, we plan to create an annual Scholarship program that can provide each
recipient with greater assistance for more advanced programs. Additionally, we will continue to provide
smaller Sponsorships as requested.
Beginning Date of Project: January 2015 End Date of Project: December 2015
Geographic Area to be served: Opportunity4Kids serves the entire Maricopa County. Our organization
does focus specifically on serving underprivileged communities and children first.

Amount of Total Project: $35,000 Amount of 12% Request: $35,000
(No Multi-Year Request will be accepted)
If the Amount of funding received is less than the amount needed for the project, do you have other
funds to complete the project? (Please Explain): Yes, Opportunity4Kids can support our project if the
full amount of funding is not provided. Our organization works year round to raise private and public
funding to support ongoing Sponsorships and Scholarships. With this generous Grant, Opportunity4Kids
would be able to help a greater population of children at a much higher level. The Grant would facilitate
larger Sponsorships and Scholarships for those that truly need assistance.

Funding for project/programs may be based on available funding. As such, if funding were not
available for the full the amount of request how would you prioritize funding.
A.) Sponsorships- These tend to be of a smaller request amount so we would grant these first to help as
many children as possible.
B.) Scholarships- These tend to be of a larger amount. We would first address the amount of funding
provided and then assess to how many children we could provide larger assistance.
