Independent Contractor Agreement
Contract No. 07012015-16
The Independent Contract Agreement dated July 1, 2015, between Arizona Community Action
Association (ACAA) and Maricopa County, administered by its Human Services
Department (hereinafter “Contractor”) to conduct application intake and eligibility
determination for utility bill assistance and deposits is hereby amended as follows:
Purpose of the Amendment:
1. To allocate additional Utility Repair Replacement and Deposit (URRD) funds in the
amount of $50,000.00 ($46,000.00 for Direct Services and $4,000.00 for Program
Delivery) in order to provide additional assistance to eligible households residing in
Maricopa County.
Amendment to:
Section I. Services and Programs – 1.2 Fund Sources:
URRD Fund Source: Increase direct service amount from $85,202.12 to $131,202.12 and
program delivery dollars from $7,408.88 to $11,408.88. The total amount of URRD
funding increased from $92,611.00 to $142,611.00.
Whole Agreement:
Unless otherwise noted herein, all other provisions of the original Agreement will remain in
place for the duration of the original Agreement.
