SECTION III WORK STATEMENT
1. BACKGROUND
Monies supporting this Agreement are funded through Federal Catalog of Federal
Domestic Assistance CFDA # 93.297. These monies are a direct Federal grant provided
to Maricopa County by and through its Department of Public Health (MCDPH) by U.S.
Department of Health and Human, Public Health Service, Office of Grants Management,,
Grant No. 1 TPIAH000129-01-00, Maricopa County Teen Pregnancy Prevention

sub recipients of federal funds are informed of the Catalog of Federal Domestic
Assistance (CFDA) title and number, award name and number, and the name of federal
agency. All Federal grant regulations for this CFDA number apply to the administration
of these funds.
2. PURPOSE
The Maricopa County Department of Public Health (MCDPH) through its Office of Health
Promotion and Education (OHPE) proposes to decrease the teen birth rate in Maricopa
County. This project will impact the teen birth rate by building the capacity of established
youth-serving organizations to provide quality, evidence-based teen pregnancy
prevention programming (TPP). The project consists of two primary strategies: (1)
targeted capacity building assistance (CBA) to at least six youth-serving organizations
within two separate cohorts and (2) the establishment of a community of practice
focused on teen pregnancy prevention.
For its part, Contractor staff members will work directly with the two cohorts of youth-
serving organizations in the following: engaging the community in implementation of
evidence-based TPP programs, assessing community needs and resources, engaging in
a formal planning process like Getting to Outcomes, pilot testing evidence-based TPP
programs, replicating TPP programs with fidelity and quality, making program
adaptations, recruiting, engaging and retaining youth, ensuring that programs are
implemented in environments that are positive, safe, supportive and healthy,
establishing and maintaining linkages and referrals to youth-friendly health care and
social services, engaging in strategic dissemination and communication activities,
collecting and using performance measures to make continuous quality improvements,
and evaluating the implementation and success of program activities. Within this
process, the Contractor staff will serve as coaches and mentors and offer peer-to-peer
support.
3. RESPONSIBILITIES OF THE CONTRACTOR
The contractor shall provide the following:
A. The Contractor will hire and supervise 1 FTE Program Lead (PL) to implement
the targeted technical assistance portion of the MCDPH Tier 1A program. This
will be completed within 30 days of the contract finalization.
B. The PL and Project Director will meet with MCDPH monthly or as needed to stay
abreast of project progress.
12

SECTION III WORK STATEMENT
C. The PL will submit monthly status reports and report assessment results and
action plans.
D. PL will meet with Pendergast Elementary School District, Isaac School District
and 1 in 10 to establish the program and obtain a Memorandum of
Understanding. Upon signature of the MOU, the site will be awarded a stipend to
offset capacity costs of $15,000 annually. MOUs will be signed yearly to
determine if all previous goals and objectives were met, to determine progress
toward sustainable capacity, and to ensure the site is still in good standing. Time
objectives will be decided in the MOU.
E. PL will conduct the OAH Organizational Capacity Assessment with the
leadership of the agency initially and every six months after and an action plan
will be developed.
F. PL will assist each site in selecting an evidence based curriculum that is a good
fit for their organizational needs.
G. By December 2015, PL will work in conjunction with MCDPH to organize
professional development trainings and curriculum trainings for each site.
H. By January 2016, PL will work with each site to schedule and pilot their
workshops. This will include facilitation of the workshops, facilitator evaluations,
administer pre/post tests, fidelity monitoring, and data entry.
I. PL will work with all three sites to have completed one complete set of EBI
workshops. This will become an on-going process. PL will provide technical
assistance, provide feedback, observe implementation, and other consultation to
ensure successful implementation.
J. By June 30, 2017, all three sites will have developed a sustainability plan for TPP
programming and will have conducted the organizational assessment three times
to measure progress. By this date, each site will have delivered at least three
series of workshops with their own staff serving as facilitators.
K. By June 30, 2017, the Contractor will ensure that all three sites are reaching
youth through the selected evidence based curriculum.
L. By June 30, 2017, the Contractor will have selected a second cohort of youth
serving organizations to participate in the capacity building process and repeat
activities in A-J through the remainder of the contract period.
