Here is what I know. I was contacted by Terri Ballard the last week of May 2015 and notified that there was a check being held at the district office for me. I had no knowledge of a check being held until that phone call. I picked up the check for $644.23 and deposited it on May 28, 2015. The check being at the district had been a result of a garnishment but I did not know that this check was at the district office until the phone call in May 2015. I do not know why I was not contacted when the check was first issued or why the check sat at the district office for over a year without someone contacting me. Had I known the check was there I would have immediately picked it up. I did suffer financial hardship during that time in 2014 and this would have helped immensely. I did not realize that the check was stale dated until after my bank issued a letter that I was being charged the $644.23 out of the account due to the stale date. Please contact me if there is further information that I can provide or if you prefer that I contact you by phone. My personal number is 480-236-6516. I appreciate your help in having the check reissued.
