GRA-STATE-16-0776-01
Between The

(First Things First)
And
Maricopa County Department of Public Health
I. Purpose
The intent of this evidence based strategy is to provide Child Care Health Consultation
services for child care centers and homes by Maricopa County. The following regions will
receive services from Maricopa County: East Maricopa, Northwest Maricopa, Phoenix
North, Phoenix South, Southeast Maricopa, and Southwest Maricopa. The CCHC will
provide health and safety information to child care staff and directors, conduct trainings,
and provide other direct or referral services related to health and safety to early childhood
providers for children birth to five. Services will be provided using the 3-tiered service
levels.
II. Term, Renewal
The term of this Agreement is July 1, 2015 through June 30, 2016. The parties may renew
this Agreement for up to two (2) additional twelve (12) month extensions (including lesser
parts thereof).
Ill. Description of Services
Early childhood professionals are entrusted to provide care for children, and they must
keep children safe from injuries and infectious diseases, while providing care. Health and
safety related issues directly impact the quality of early care and education programs. The
health and safety standards are based on the Caring for Our Children (3rd edition)
standards that are higher than state licensing standards. The Child Care Health
Consultation program is evidence based and has been shown to promote healthy and safe
improvements in child care environments.
Child Care Health Consultants offer specialized training and technical assistance
to child care staff and directors of centers and homes enrolled in the Quality First Program.
In some regions, there are CCHC only centers that are not enrolled in the Quality First
Program. The role of the CCHC is to promote the health and development of children,
families, and to ensure a healthy and safe child care environment. Topics of technical

assistance: sanitation, hand-washing, playground safety, medication management,
emergency preparedness, and health and illness management.
The target population of the Child Care Health Consultation Program is centers and homes
enrolled in the Quality First Program, and in some regions in centers or homes not enrolled
in Quality First as determined by the regional council.

Required Narrative Responses
Provide a narrative response that addresses the following items.
a) Provide a description of the CCHC program being implemented in your region(s).
Child Care Health Consultants provide tiered health consultative services to all regions of
Maricopa County in both regulated child care center and homes who participate in Quality First,
the Quality Improvement and Ratings System developed by the Early Childhood Development
and Health Board. The design of the child care health consultative program that is utilized is
influenced by the work of the National Training Institute for Child Care Health Consultants out of
the University Of North Carolina at Chapel Hill. The CCHC program is a voluntary, helping
process which takes place between the CCHC and the child care provider. It is a problem solving
process with the intent of addressing immediate concerns in health and safety areas and
Euevere-pinvexpE are LH L1V (le mar concern arises in e
future, the provider will be able to handle it independently. In addition, it is a fully collaborative
process which includes not only the child care program director and staff, but also other OF
Technical Advisors, such as OF coaches, inclusion coaches, Mental Health Consultants and OF
education specialists. For the Fiscal Year 16 the CCHC program will be staffed with 1 nurse
supervisor, 7.5 Registered Nurses, and 1 administrative assistant.
b) Identify capacity or infrastructure building, which will be needed, including agreements and
partnerships with other departments and agencies, additional resources, and training and
technical assistance to provide the proposed service.
The infrastructure for the CCHC program housed at Maricopa County Department of Public
Health (MCDPH) is and has been in place for a period of five plus years. The CCHC program, as
part of Community Health Nursing, partners with other areas in the MCDPH Disease Control
Division, such as Office of Epidemiology, Office of Preparedness and Response and Office of Vital
Registration. Community Health Nursing has partners throughout the Department of Public
Health in the Community Health Action Division which houses the WIC program, Office of Family
Health, Office of Oral Health, and Office of Health Promotion and Education. In the MCDPH
Community Transformation Division there is the Office of Tobacco & Chronic Disease, Office of
Nutrition and Physical Activity, Office of Community Empowerment and the Office of Public
Policy. Being part of the MCDPH we also partner with the nonprofit organizations Maricopa
County Children's Immunization Partnership(MCChIP) and The Arizona Partnership for
Immunization (TAPI). The Child Care Health Consultation Program also partners with other
agencies under the umbrella of First Things First such as the Valley of the Sun United Way,
Southwest Human Development, and Arizona Association for Supportive Child Care working side
by side with Coaches, Mental Health Consultants, and Inclusion coaches in a collaborative
capacity. Arizona Department of Health Services is another partner we work with especially the
Office of Child Care Licensing. All CCHC's go through a Nil CCHC certification course upon
getting hired as a CCHC at MCDPH. They also attend numerous trainings such as Owner/Director
Orientation Workshop sponsored by ADHS Office of Child Care Licensing and Arizona
Department of Economic Security Family Child Care recruitment training. There are many
trainings/conferences that focus in on the area of child care that are offered to the nurses
throughout the year (Obesity conference, Child Abuse conference, AZ Annual Immunization
conference, FTF Summitetc.) The CCHC's participate in the Quality First Coaching Academy
alongside other collaborative partners in Quality First to gain knowledge of the multi-stage
coaching model identified for use by FTF when offering technical assistance to child care
providers. The OF Coaching Academy also promotes professionalism and cultural responsibility.
c) Identify barriers to providing the service or program proposed and plans for addressing these
barriers.
Regional councils have budgeted for a specific number of CCHC's to cover all Maricopa County
QF Child Care Centers. There are a number of factors that influence the ability to effectively
manage a caseload and a point system has been put into place(See CCHC Handbook pg. 26). The
maximum caseload for each CCHC FTE is 25 clients not to total more than 30 points. Utilizing the
CCHC Tier Model for services, we are typically able to provide Tier 2 and 3 services to
approximately 45% of the total number of QF programs. Services will continue to be provided
via the CCHC Tiered Services Model.
A barrier or challenge for this grant cycle will center on Tier One Programs. Since the Helpline
will no longer be available to provide Tier One services, this task will fall to the MCDPH CC-IC
program in the new fiscal year. All Tier 1 Programs will be contacted to see if they need
services and will have be given access to a central phone number/e-mail to contact if they
decide they would like services. The CCHC's will provide Tier 2 and 3 level services to those
programs that choose to participate at the Tier 2 or 3 levels.. If the demand for Tier 2 or Tier 3
CCHC services outstrips the number of CCHC's we have available to provide the services, then
we will implement a waiting list protocol. The protocol will rotate "wait list" programs on to a
CCHC caseload as other Programs are moved down to the Tier 1 level, thus opening up a space
for the "wait listed" program. Several factors determine as to which Programs stay on service
or move back to Tier 1. The first factor is the commitment a center makes to participate in the
OF CCHC Process (e.g. keeping appointments, allowing CCHC to visit on at least a quarterly basis,
returning on going communication). Programs that are not able to make this commitment for
CCHC services can be moved to Tier One Services level until they are ready for CCHC services.
Other factors that keep the Tier System fluid could include considering the length of time a
program has been receiving CCHC services (especially any program that have been receiving
services for 3 or more years)„ or programs that have made marked improvement (3-5 stars).
These programs could move down to Tier 1, thus allowing others to have Tier 2 or 3 CCHC
services. For those improved programs that desire to continue with a CCHC, but that do not
require Tier 2 or 3 services, we would give them the opportunity to be assigned to the CCHC
they have been working with to maintain that excellent working relationship, but at Tier One
CCHC service level.
In order to provide additional services to Tier One programs, the MCDPH CCHC program
proposes that Tier one programs have access to a central MCDPH CCHC line to request CCHC
services. The MCDPH CCHC Program already has on-line training in place for child care
providers, in addition to community trainings throughout the year, both of these training
offerings are available on our web site: http://www.maricopa.govipublichealth/defaultaspx
A second barrier to providing services is the sheer number of collaboration meetings that the
CCHC must attend quarterly as well as the commitment to attend the OF Academy meetings
numerous times throughout the year . Such meetings take time away from providing direct
services to programs. To address this barrier, we are having the CCHCs utilize other means of
communication with collaborative team members such as email/telephone instead of having to
attend every meeting in person. The CCHC's are also contributing to the collaboration process
for the quality improvement plan by writing health and safety SMART goals and sharing with the
team.
d) Describe plans to recruit and locate personnel within the geographical region of the provided
service that meet the staff qualification standard detailed in the Standards of Practice, and are
linguistically appropriate and culturally responsive for the population to be served. If there is
anticipated difficulty in hiring qualified personnel, include a plan and timeline for supporting
staff to meet the qualification standard.
Currently, the MCDPH Child Care Health Consultant Program is fully staffed with 7.5 FTE CCHC's
and 2 supervisory staff. As discussed above, the staffing model will change in the new fiscal year
to 1 supervisory staff and 7.5 FTE CCHC staff and one FTE administrative assistant staff. Should
staffing levels change, due to staff turn-over, MCDPH has hiring mechanisms in place through
DLIT hUIrIdFl ResoulLes uv nsure that qualitied candidates are selected tor the
interview process. Job descriptions are reviewed annually and updates are made according to
grant requirements. At this time, MCDPH CCHC Program does not anticipate difficulty in hiring
qualified staff.
e) Describe steps that will be taken to promote collaboration with other government departments
and partners working with the agency.
Collaboration networks with other government departments and partners working with the
agency are already in place with the capacity to expand these networks as needed.
1) The CCHC program collaborates with the other Quality First technical advisors working with
Quality First Programs on several different levels: Individual program collaborative efforts to
develop the Quality Improvement Plan, CCHC staff attendance and participation with the
Quality First Academy, and Supervisory participation in joint collaboration meetings and at
quarterly VSUW Coaching meetings. In addition, BASECAMP allows for CCHC staff and
supervisors to pose questions to the assessment team and to clarify the meanings of
standards taken from the Environmental Rating Scales and Caring for Our Children, 3rd Ed.
CCHC's also collaborate with all pertinent technical advisors to address critical incidents that
may arise with a OF program.
2) The CCHC program maintains collaborative relationships with ADI-15 Office of Child Care
Licensing and DES, with members from those offices invited annually to a CCHC Statewide
meeting. CCHC's have the capacity to contact ADHS for questions and clarifications
regarding licensing regulations.
3) The MCDPH CCHC program and MCDPH Office of Nutrition and Physical have partnered with
ADHS Bureau of Nutrition and Physical Activity to implement programs aimed at obesity
prevention in the child care setting through the Empower and Empower Plus Programs.
CCHC's focus on helping child care programs implement policies from the Empower Program
and demonstrate how Empower and Empower Plus concepts can readily be adapted to their
individual programs.
4) The MCDPH CCHC Program collaborates with Pima County CCHC Program to determine
dates/sites of CCHC Training Programs and Health and Safety Specialist Training Programs.
The two programs also meet to discuss overall curriculum of the CCHC and HSS training
programs to ensure that content is standardized for both training programs.
5) The MCDPH CCHC program also collaborates with our internal MCDPH departments.
Community Health Nursing (CNN) has county funded CCHC's who help support non-QF
programs in Maricopa County. CHN also provides NTI certified trainers who help produce
CCHC Training that is done by MCDPH CCHC program The CCHC program partners with the
CHN program to provide free adult immunizations to child care providers following
community training events. The office of Epidemiology will alert the CCHC program of any
reports of outbreaks in a child care center. If the program is a OF program, then a QF
funded CCHC can contact the program to assist the program in managing their outbreak.
The CCHC program has partnered with our Public Information Office to create on-line
trainings for child care providers on the CCHC section of the MCDPH web site. Plans are in
place to expand this partnership to include additional training offerings and to put up a
Facebook page to reach more child care programs with pertinent information.
f) Describe the plan and resources necessary to meet FTF basic reporting requirements and
maintain data securely and confidentially.
I ViCUP program TrUgT2 aireaay nas rc=lciurs in tnat meet tne F Basic Ke po rung

the secured FTF extranet and PGMS system. Maricopa County Office of Enterprise Technology's
(OFT) Information Security division provides the processes and technology to ensure the
confidentiality, integrity and availability of all County information systems, to include those of
MCDPH. The Information Security organizational unit focuses on information security policies,
governance and compliance. All staff members are required to review Maricopa County and
Maricopa County Department of Public Health Ethics and HIPAA policies. MCDPH has additional
polices in place for HIPAA compliance and for maintenance of hard copy records and storage of
closed records.

Personnel Services: $695,190
Salaries for 8.0 FTE CCHCs, 1.0 FTE CCHC Program Assistant, and .80 FTE CCHC nursing supervisor.
Maricopa County Department of Public Health uses a Placement in Range System for determining
nursing salaries using an average rounded salary and salaries are determined based on the number of
years of nursing experience. Most of the nurses are very experienced and earn a salary in the top range,
although some earn less. The number of supervisors will decrease this coming fiscal year to one
supervisor and the number of CCHCs will increase to 8.0. The 8.0 FTEs are budgeted using an average
rounded rate ($35,54 x 2096 hours x 8.0FTEs= $595,946). Please note, average rounded rates are being
utilized to reflect Maricopa County salary increases that were put into effect mid —term FY 2015-2016
and an anticipated average salary increase for Maricopa County employees during FY 2015-2016, thus
accounting for overall budgeted salary increases over last fiscal year.
A $626 language differential is budgeted for each of the 2 employees who are bilingual in English and
Spanish. The additional .25 FTE added for the CCHC Program assistant will reflect in increased
administrative duties that reflect in the implementation of the new grant. The supervisor's salary is
budgeted based on the salaries of the current incumbent, for a total of $67,786. The supervisor's salary
is shared with another grant for technical assistance therefore there is .80 FTE in this grant and .20 in
the TA grant. The supervisor will be responsible for the day to day supervlson of staff, determining
staff assignments within the tiered services delivery model, collecting and reporting all data as required
by FTF regarding program activities, providing recommendations for CCCHC system development, and
fostering collaboration between CCHC's and other early childhood coaches and consultants.
