These youth and their families will be referred by an in-house medical clinic, operated by Mountain Park Community Health Center. The addition of fitness programming will create an integrated, “one-stop” wellness collaborative. a. Need and how need will be addressed: According to a 2012 Maricopa County community assessment, obesity and type 2 diabetes are the top two health priorities. These conditions are of even greater concern within the County’s large, low-income, Hispanic population. Alarmingly, obesity and pre- diabetes rates are growing quickly among young Hispanics. Both of these conditions are correlated with lack of access to preemptive medical care, poor nutrition, and physical inactivity. The Legacy Foundation Chris-Town YMCA and Mountain Park Community Health Center are in a unique position to address these County health concerns with a seamless integration of financially accessible healthcare, wellness, and fitness programming targeted at low-income Hispanic youth and their families from the Chris-Town neighborhood. Mountain Park Community Health Center has recently signed a 5-year lease with the Legacy Foundation Chris-Town YMCA to operate a pediatric medical clinic (Abrazo Health Care has operated a clinic at this location since 2011). The clinic provides comprehensive pediatric primary care. For those who don’t have insurance or are underinsured, the clinic offers full access to healthcare. Staff help patients enroll in AHCCCS or the clinics own sliding fee program. An onsite dental clinic works cooperatively to provide oral care. Our Legacy Foundation Chris-Town YMCA provides the medical clinic with an on-site fitness venue and trained fitness professionals. Doctors and other clinic professionals prescribe safe, appropriate, medically-based exercise, and then work with YMCA staff to 1 ensure successful, long- term integration into the youth’s life. Since our collaborative realizes the youth’s family must be incorporated in order to spur lasting behavioral change, the whole family unit is encouraged to engage in exercise. The Legacy Foundation Chris-Town YMCA is the only YMCA in the country to integrate a medical clinic (as well as a dental clinic) into its facility, and continues to be a leader and innovator in this comprehensive approach to wellness for underserved communities facing high rates of life-style related disease. b. Population served/specifics of services provided by Legacy Foundation Chris- Town YMCA: Approximately 90 children are seen at the clinic each year. Roughly 60% (54) of these children have or have some risk factor for type 2 diabetes or other life-style related chronic disease and warrant a medical professional’s prescription for physical activity. These children and their families are offered a 12-month membership to the Legacy Foundation Chris-Town YMCA. Clinic medical professionals work with YMCA staff to implement a prescribed fitness routine for the entire family unit. Family members receive guidance on beginning a fitness program through our “Smart Start Program.” This program begins with a 15-minute individual appointment with our “Smart Start Concierge.” During this appointment, a fitness plan is devised referencing the clinic’s exercise prescription. This fitness plan may include group fitness classes (which range from water fitness to circuit training to Zumba) or an appointment with a fitness attendant to become acquainted with cardiovascular and/or strength training equipment. The “Smart Start Concierge” then follows up with family in one week, inquiring about their satisfaction with the “new routine.” After this, monthly check-ins take place to revise, modify, or reinvigorate fitness plans. Communication between clinic medical professionals and YMCA fitness staff is continuous to ensure safety, appropriateness, and effectiveness of programs. Children 12 and over utilize fitness floor and attend group fitness classes with parent or guardian. Youth 14 and over utilize fitness floor and attend group fitness classes without parent or guardian. Families also utilize gym and pool as activity venues. Childcare is available for young children (6 months through 11 years). This childcare includes fitness/wellness and educational enrichment programming (including homework help), fostering physical and intellectual 2 development. Healthy afterschool and summer meals (breakfast and lunch) are also available free of charge to children 18 and under through a partnership with St. Mary’s Food Bank. In addition to these membership fitness opportunities, specialized classes are provided for families referred by the clinic. Twice per week for one hour, a children’s fitness class is held. During this time, adults engage in group fitness classes and other facility fitness opportunities, as described above. Once per week for one hour, a family fitness class is held, drawing all family members together. These classes range from Zumba—families learn that dancing is a fun, easy, and inexpensive way to workout together—to obstacle course challenges, where families enjoy running, jumping, crawling, and competing with other families. The Legacy Foundation Chris-Town YMCA clearly offers a wide range of activities to engage family members of all abilities, ages, and health statuses in fitness. But the Legacy Foundation Chris-Town YMCA is more than just a fitness center—it is a place for families to connect, to find encouragement, and to belong to a community—all essential factors in creating lasting life-style change and optimal wellness outcomes. c. Budget: A total of 54 family memberships will be provided. These memberships will give the whole family unit (parent(s) and all children) full access to all above described fitness and wellness services. Based on demographics these memberships will be broken down as such: Family 1: $780/year x 18 single parent families= $14,040 Family 2: $1,020/year x 36 two parent families=$36,720 YMCA Staffing for specialized fitness classes: 48 weeks x 3 one-hour classes per week x $20/hour for instructor salary and fringe=$14,400 Total Request=$65,160 Please note: Program can be implemented for fewer families if only limited funds are available. Specialized fitness classes can also be implemented as a solo project. 3 d. Other funding requests: As of this time, no other requests for funding have been made for this project. e. 12% distributions already received: Legacy Foundation Chris-Town YMCA received a distribution of $250,000 in 2013 from the Gila River Tribe to support its dental clinic. No distributions have been granted from the Tohono O’odham Nation. f.
