I. The above named contract is hereby amended as specified below:
A. Cover Page, under 1.0 Contract Term, 1.1., the original contract shall be extended for an
additional term of one (1) year, beginning on August 1, 2015 and ending July 31, 2016.
II. All other terms and conditions of the original contract and its amendments shall remain in
full force and effect.
