AMENDMENT No. 5 to LEASE AGREEMENT
Between
TOWN OF GUADALUPE, LESSOR
And
MARICOPA COUNTY, LESSEE
RECITALS
A. Lessor and Lessee are Parties to that certain Lease Agreement No. L-7309 dated and
approved by Maricopa County Board of Supervisors and subsequently amended March 9,
2005, May 11, 2011, August 31, 2011 and April 24, 2013 (the “Lease Agreement”). The
Agreement is for premises at 9401 South Avenida del Yaqui, Guadalupe, AZ, consisting
of 2,364 square feet of classroom, kitchen, and restroom space for Head Start Program
use.
B. The term of the Lease Agreement expires on May 30, 2015.
C. Lessor and Lessee now mutually desire to amend the Lease Agreement to extend the term
of the Lease Agreement, update Lessor notice, update payment address and Lessee notice
address, and add janitorial service to the Lease Agreement.
AGREEMENT
NOW THEREFORE, in consideration of the foregoing and other valuable consideration, the
sufficiency and adequacy of which the Parties acknowledge, Lessee and Lessor agree to amend
Lease No. L-7309 as follows:
1. The term of the Lease Agreement is hereby extended to May 31, 2019. This Lease
Agreement is subject to termination pursuant to the provisions of A.R.S. §38-511.
2. All notices herein required to be given to Lessor and all payments herein required shall
be sent to Lessor at:

3. All notices herein required to be given to Lessor and all payments herein required shall
be sent to Lessor at:

4. Lessor shall provide janitorial services to the two (2) Premise restrooms twice weekly at a
cost of no more than Thirty Dollars ($30.00) per week.
5. The effective date of this Amendment No. 5 is the date of execution by the Maricopa
County Board of Supervisors.
6. The foregoing paragraphs contain all the changes made by this amendment. All other
terms and conditions of the original Agreement and as modified by subsequent
amendments shall remain the same and in full force and effect.
