I. The above named contract is hereby amended as specified below:
A. Expiration Date is extended to December 31, 2015.
B. Contract Amount is increased by $49,999.98 to an aggregate not-to-exceed
(NTE) total amount of $651,999.98 for the budget period of July 1, 2015 through
December 31, 2015.
II. All other terms and conditions of the original contract shall remain in full force and
effect.
