Wellness Premium Incentives
The following requirements are effective for benefits Plan Year 2015-16 (July 1, 2015 through June 30,
2016) and supersede all previously approved program requirements.
Biometric Screening and Health Risk Assessment:
Employees will receive a reduction in their ongoing medical premium of up to $40.00 per
month if they complete both the Biometric Screening and the on-line Health Assessment
offered by Maricopa County through its approved wellness vendor.
Non-Tobacco Use:
Employees who are enrolled in a County-sponsored medical plan and have completed both the
Biometric Screening and Health Assessment will receive a reduction in their ongoing medical
premium of up to $20.00 per month if they do not use tobacco products. In order to qualify
for the premium reduction incentive, employees must:
1. Attest that they have not used tobacco for at least six (6) months.
2. Pass a County-sponsored screening to detect tobacco use. If they have successfully passed
the screening for tobacco use in the past, employees will not need to be screened again for
Plan Year 2015-16. Employees who do not use tobacco products but who are otherwise unable
to take or pass the tobacco screening for medical reasons may request a waiver of this
requirement with documentation from their healthcare provider.
As a reasonable alternative, employees who are tobacco users will receive the medical
premium reduction if they participate in and complete a Tobacco Cessation Program approved
by the Department of Employee Benefits and Health that requires significant effort and
includes one-on-one health coaching.
Prospective Payment of Wellness Premium Incentives:
Beginning with Plan Year 2015-16, Wellness premium reduction incentives will be paid out prospectively
once an employee qualifies for them.
Continuation of Plan Year 2014-15 Wellness Premium Incentives:
Employees who qualify in Plan Year 2014-15 for wellness premium reduction incentives for biometric
screening, health risk assessment and non-tobacco use who elect to participate in Plan Year 2015-2016
County-sponsored medical plans to continue to receive such Plan Year 2014-2015 incentives until they
complete the requirements to qualify for Plan Year 2015-2016 incentives or September 27, 2015,
whichever comes first.
