CAREFREE HWY, PMB #385 PHOENIX, AZ 85085 PHONE: 623-465-7400 FAX: 623-465-7632 TOHONO O’ODHAM NATION GRANT PURPOSE OF THE GRANT The proposed grant will directly benefit the Daisy Mountain Fire Districts (DMFD) customers, employees, budgets, and automatic/mutual aid recipients. The purpose of this grant is to purchase a community support services van to assist and progress our current community services. The proposed grant is to purchase and equip a partially funded 2016 Chevrolet 2500 Express 12 Passenger Van. The community support services van will be a multi-purpose support vehicle that will be used in a variety of programs. Daisy Mountain Fire District is currently working with local hospitals to implement our Community Paramedic Program. This program is a vital necessity for our citizens, fire department, and local hospitals. The Community Paramedic Program expands our role as Emergency Medical Service (EMS) workers on a non-emergency medical unit that can assist with the overburdened emergency rooms at local hospitals by providing follow up care with scheduled house calls. Similar initiatives in the United States and around the world have generated remarkable results with post-op wound care, medication/prescription follow-up, disease management, and prevention by providing general check-ups. This program can provide health services where access to physicians, clinics and/or hospitals may be difficult due to the rural setting and/or patient transportation issues. This grant gives us the vehicle to be able to start providing these services to serve the needs of our particular rural communities. The Daisy Mountain Fire District is also seeking to provide our communities and those communities within the valley wide automatic/mutual aid system with a Crisis Response Team. A Crisis Response Team is a dedicated team of highly trained staff or volunteers who assist people with practical and emotional needs following traumatic events. The Crisis Response Teams are dispatched in conjunction with Fire and/or Police emergency units to provide crisis intervention, support and resource referral services. A Crisis Response team is able to fully focus on the emotional and social needs of the family and/or individual in crisis while the Police and/or Fire personnel attend to law enforcement, medical emergency, and fire suppression needs. By providing these valuable social services, this unit enables Fire and Police personnel to return to their normal operational duties quicker. This passenger van will be able to provide the vehicle necessary to implement a Crisis Response Team. In addition to these programs this passenger van, when available, can assist in a multitude of community and member services. Our department has recently implemented a Community Services Program Coordinator that is responsible for developing and implementing a variety of public education and volunteer programs for the Daisy Mountain Fire District. The Community Services Program Coordinator will be instrumental in ensuring the success of our community programs we currently offer as well as our future programs. These programs include, but are not limited to; Car Seat inspections, CPR/AED/First Aid, CCC, Fire Pal, Water Safety, Senior Outreach issues, Muscular Dystrophy Fundraisers, Daisy Mountain Firefighters Charities, Crisis Response Team, Community Emergency Response Team and Fire Corps volunteer programs. Furthermore this passenger van can assist with our members by transporting multiple members to yearly physicals, required continued education trainings, and/or voluntary professional development trainings. By having the 12 passenger van available, we can reduce the mileage placed on our high dollar apparatus, reduce our vehicle emissions, and reduce fuel costs when multiple members need to travel for these occasions. BENEFITTING POPULATION Our Fire District is 107 square miles of coverage area located in Northern Maricopa County. Our Fire District serves 45,000 residences in the communities of Desert Hills, Anthem, and New River. Daisy Mountain is also part of the valleys automatic/mutual aid system where we are utilized to assist in any emergency in the valley when resources are needed. The Community Paramedic Program will not be limited to our fire district, due to its non-emergency status, and can serve an unknown number of citizens in our surrounding communities. The Crisis Response Team can also serve as a vital resource for the millions of citizens in our automatic/mutual aid communities. This just reiterates that the proposed passenger van can serve as a critical health care and public safety resource. PROPOSED BUDGET The Daisy Mountain Fire District is requesting a $31,447.53 grant toward the purchase of a 2016 Chevrolet 2500 Express 12 Passenger Van, department decals, and radio equipment for communications. The total cost of this passenger van ($28,447.53), decals ($1,000), and radio equipment ($5,000) is $34,447.53. Daisy Mountain Fire District is committed to covering any costs exceeding the $31,447.53, being requested for this grant, in the amount of $3,000. Any other costs of maintaining and operating this apparatus will be the responsibility of the Daisy Mountain Fire District. Daisy Mountain Fire District prides itself on providing quality fire protection, medical services and community services to our community, especially during these difficult economic times. For the past five years, DMFD has experienced an economic downturn and reduced its budget by over ten percent, while experiencing a capped 5% increase in operational costs each year as a result of proposition 117. The DMFD is funded through property taxes. We receive our funding through a secondary property tax assessment levied on properties within our district boundaries. The levy rate has a State of Arizona mandated cap thresh hold and we have reached its cap, therefore cannot increase the levy rate to compensate for the decrease in funds through the property tax. It is well documented that Arizona, specifically the Maricopa County Area, has over the past five years had decreases in property values of over 69.6% (source: Maricopa County Assessor’s Office). The housing market has begun a slow recovery but the DMFD will not see any significant increases in revenue for at least two years due to the tax collection process in Arizona. As a result DMFD is still recovering from the downturn in the economy. OTHER FUNDING RELATING TO PROPOSAL Daisy Mountain Fire District has not pursued any other funding requests or any substantially related proposals for a community support services van. 12% DISTRIBUTION FROM ARIZONA TRIBES The Daisy Mountain Fire District has not received a 12% distribution from Tohono O’odham Nation. Our district did receive a 12% distribution to replace an aging 1991 brush truck from the Gila River Indian Community in the amount of $250,000, which was awarded on October 15, 2014.
