UMOM New Day Center, Inc.
3333 East Van Buren, Phoenix, AZ 85008.
I. The above named agreement is hereby amended as specified below:
A. Section F. IT IS MUTUALLY UNDERSTOOD AN AGREED BY AND BETWEEN
THE PARTIES THAT, under paragraph 3. TERMINATION. Either Party, in
writing, may terminate the instrument in whole or in part at any time before the
expiration. MCDPH WIC Services will be exercising this option to terminate the
instrument in whole.
II. All other terms and conditions of the original agreement and its amendments shall
remain in full force and effect.
